SCOPE OF WORK

Handrail Replacement CLC Buildings 4, 7, 11, 91, 94 (Waco)

Requirements The contractor shall be responsible for providing all labor, materials, and equipment needed for the services required to demo and install new Handrail in Buildings 4, 7, 11, 91, 94 (Waco). Work performed will be done at the Central Texas Veterans Health Care Facility in Waco, at 4800 Memorial Drive, Waco, Texas 76711. After all work is completed, the contractor will have a walkthrough with the VA COR or Alternative COR to ensure all work is completed and ready to receive payment.

The contractor shall provide all personnel, services, vehicles, transportation, tools, equipment, materials, supplies, facility supervision, administration supervision and any other item(s) necessary to perform and accomplish this service. The contractor shall be familiar with all the terms, conditions, and requirement herein contained. All work must comply with federal, state and local regulations.

Handrails provided shall be new brand name or equal to 1000BH Handrail - Ligature Resistant Handrails | Inpro Corporation, preferred color Santa Rosa Oak. See attach cut sheet for specifications (attachments 3 and 4). The approximate length required for all building is approximately 5714 linear feet (LF) of railings. The break down per building is as follows:

The dimensions provided here are approximate in nature and the contractor shall be responsible for verifying all measurements prior to awarded any work. Failure to verify or discrepancy in measurement will NOT constitute an authorized change.

Handrail Salient Characteristics:

- Ligature resistant handrail (attachment 3)

- Scratch and stain resistant (attachment 3)

- Easy to clean and able to withstand harsh cleaning agents such as bleach and degreasers

(attachment 3 and 6)

- Continuous retainer/bracket for enhanced safety (attachment 3)

- Handrail measurements, in accordance with “ADA Standards for Accessible Design” 4.26 and

4.27 from 28 CFR Part 36 (attachment 12)

- Includes limited lifetime warranty on product (attachment 5)

All work shall be done according to the Infection Control Risk Assessment (ICRA) specified for the respective areas where work is performed.

Note: “The C&A requirements do not apply, and that a Security Accreditation Package is not required for this contract.”

Period of Performance (POP) = 120 days from the Notice to Proceed

Project Requirements Base Bid – Demolition Phase

The contractor will be responsible for the following during this phase.

1. The contractor shall provide a project schedule prior to beginning any work and coordinate with

COR on a regular basis.

a. For buildings 4, 7, 11, and 91 work shall be scheduled to be after hours for non-housing unit areas. After hours is defined as Monday to Friday from 5:00pm to 6:00am or on Saturday or Sunday from 7:00am to 5:00pm. Work in housing units must be conducted during regular business hours which are defined as 7:00am to 5:00pm.

i. Special care must be taken while working in housing units by providing barriers that will prevent intrusion by patients that may be impaired visually or have cognitive issues.

ii. All work shall be performed in housing units with the lowest impact possible.

iii. A full-time VA escort will be assigned when working in housing units to assist with patient and staff diversion.

b. For building 94 all work shall be conducted during normal business hours to insure assistance with in-care patience from VA staff.

2. Contractor shall cordon off areas of work using cones, flagging and signs to prevent inadvertent entry by patience and staff.

3. Prior to conducting any work in the locked ward of building 94 the contractor shall coordinate with the COR, security personnel and hospital staff. At minimum the following daily security task must be performed while working in this area:

a. Provide a daily checklist of all tools brought into the secure area and account for the same tools at the end of the workday.

b. Maintain control and dispose of construction waste as it is generated.

c. Only bring in the new materials once ready to be installed.

d. Never leave tools, power cords or other materials unattended or unsecured.

e. Should a tool, equipment or material become unaccounted for the contractor shall notify staff, security, and COR immediately.

4. Provide low impact work that will not interfere with building/floor operations. Any work creating dust and debris shall be cleaned as work progresses.

5. Removing all handrails in the area being worked. Patch, repair and paint areas that are damaged during handrail removal.

6. Dispose of all demoed materials of off sight and in accordance with local state and federal guidelines.

Base Bid – New Work

The contractor will be responsible for the following during this phase.

1. The contractor shall provide material sample to the COR prior to any purchase.

2. The replacement handrails are to be like or equal to

3. All handrail installation shall be conducted in a low impact manner and should not interfere with the daily operation.

4. The same housing unit and building 94 requirements as described in the demolition section shall be followed during the installation of the new handrails.

5. Any damage to the facility caused by the contractor shall be fully repaired to match preconstruction conditions at the contractor’s expense.

Note: At the end of each workday, please clean-up all worked in areas.

1. Contractor will contact VA COR or Alternative COR prior to start of the work.

2. The Contractor shall schedule work through the COR, to ensure that all necessary parts, materials, tools are accounted for prior to the start of work.

3. The contractor shall coordinate with VA COR NLT than 10 days to the start of this service and completion. See information below:

A. Government Holidays:

(a) The Government hereby provides notice and Contractor hereby acknowledges receipt that

Government personnel observe the listed days as holidays:

(b) In addition to the days designated as holidays, the Government observes the following days:

a. Any other day designated by Federal Statute

b. Any other day designated by Executive Order

c. Any other day designated by the President’s Proclamation

It is understood and agreed between the Government and the Contractor that observance of such days by Government personnel shall not otherwise be a reason for an additional period of performance, or entitlement of compensation. In the event the Contractor's personnel work during the holiday, they may be reimbursed by the Contractor, however, no form of holiday or other premium compensation will be reimbursed either as a direct or indirect cost, other than their normal compensation for the time worked.

4. Submit the proper documentation and reports to receive payment on this service. See information below:

A. Invoices and Payment

a. Payment to be made monthly, in arrears, following satisfactory performance by Contractor’s personnel and upon receipt of a properly prepared invoice.

b. Contractor shall be required to invoice through the Tungsten System at http://www.tungsten-network.com/us/en/veterans-affairs/. Additional information regarding Tungsten shall be provided upon award.

c. The contract shall have current registration in the System for Award Management (SAM) for this solicitation; URL: www.sam.gov.

d. The invoice MUST be itemized to include the following information. Any information listed below, and not provided on an invoice, will render that invoice incomplete and the invoice will be returned for immediate correction.

i. Time period being invoiced for.

ii. Description of services performed.

iii. Unit Cost billed.

iv. Extended amount due.

v. Invoice number, date, and.

vi. Contract and obligation numbers.

Other Considerations

• The contractor shall be responsible for the verification of all requirements for this project. All measurements and conditions must be verified by the contractor. Any discrepancies between field conditions, project documents, and this SOW shall be immediately brought to the Contracting Officer’s attention.

• The contactor must be qualified/certified to painting repair procedures in accordance with all local codes/ regulation.

• The contractor will be required to submit a safety plan for VA review and approval prior to the performing work.

• The contractor must comply with the requirements for Type C/Class II (Lowest Risk) Infection Control Risk Assessment (ICRA).

• It is the contractor’s responsibility to provide adequate documentation verifying that any equipment and/or materials proposed will meet or exceed all federal government requirements.

• Majority of the work shall be performed after hours for buildings 4, 7, 11, and 91. After hours are described as 5:00pm to 6:00am Monday through Friday and 7:00am to 5:00pm on Saturday or Sunday. Work performed in building 94 will be conducted during normal working hours unless other wised noted by the COR. Normal working hours are 8:00 am – 4:30 pm, Monday – Friday.

• The contractor’s supervisor shall always be available to address any needs VA COR/Alternative COR may have during the period of performance which is120 days from start to finish and alert the Alternative COR of daily progress.

• The contractor shall thoroughly clean inside and outside of project site daily with contractor’s own equipment, including vacuums, mops, buckets, wash rags, dust mops, etc.

• The contractor shall be responsible for the removal of project demo materials/debris /equipment from this facility when complete.

Codes and Standards The project shall be in accordance with the latest International Building Code (IBC), National Fire Protection Associate (NFPA) standard, Uniform Federal Accessibility Standard (UFAS), VA Specifications and Standards, and local facility requirements. In the event there is a conflict between any of the standards/codes, the more stringent standard/code shall apply.

The contractor shall provide all applicable safety signage and barricades applicable to OSHA, VHA and DOD regulations. Contractor shall comply with VHA DIRECTIVE 2004-012 and all VA Center Policy Memorandums, including but not limited to the following:

(a) Protection of patients, visitors, and employees from traumatic injury, as well as occupational and facility–associated infections.

(b) Compliance with OSHA and State construction safety regulations. Construction contracts require that the general contractor’s on-site “competent person” have completed the OSHA 30-hour construction worker course and that all other general and sub-contractor’s workers have completed the OSHA 10-hour construction worker course or other relevant competency training. The determination for training is based on the project hazards and complexity, State and Federal regulations and VA requirements. Verification of completion of training will be required prior to performing work.

(d) Compliance with EPA and State environmental regulations.

SAFETY PLAN REQUIREMENTS

Prior to performing any work, the contractor will be required to submit a safety plan in accordance with OSHA and VA Regulations.

The safety plan shall include, but not limited to the following:

A. Safety and Health Policy (i.e., company policy) B. Safety and Health Objectives (i.e., company goals, procedures, etc.)

C. Job Site Inspections (i.e., safety inspection/potential hazards/prevention, etc.)

D. Contractor Safety Administrator (i.e., competent person/safety officer) E. Personal Protective Equipment (i.e., head, hearing, and eye protection, footwear, clothing, gloves, etc.)

F. Safety Rules (i.e., rules applicable to the work/task being performed, equipment, ladders, confined spaces, open flame, etc.)

G. Job Safety Training (i.e., OSHA, safety meetings, etc.)

• 30 Hour OSHA Construction Safety Course – competent person

• 10 Hour OSHA Construction Safety Course – all others

H. Safety Discipline (i.e., correction of violations, etc.)

I. Emergency Procedures (i.e., responsibilities, procedures, etc.)

J. Written Hazard Communication Program (i.e., responsibilities, procedures, list of hazards, MSDS, etc.)

SAFETY INSPECTIONS

On a weekly basis, the VA will conduct a safety inspection of the work area. This inspection includes, but is not limited to the following:

A. Means of Egress

• Fire/Smoke Doors unlocked, free of obstructions.

• Means of Egress (includes Exits) free of obstructions and clearly marked.

B. Fire Alarm, Fire Detection, and Fire Suppression Systems.

• Fire Alarm Pull Stations are unobstructed.

• Smoke Detector smoke caps in use or removed at end of the day.

• Sprinkler Heads are unobstructed.

C. Housekeeping and Traffic Control

• Trash and debris are removed promptly and transported outside the construction area.

• Floor mats and dust tack mats are located at the entrance to construction area.

• Tools and equipment are properly stored.

• Signs installed to restrict patient access (doors locked as needed)

• Dust control measures are in place.

• Barriers are in place and properly sealed.

D. Hazardous Chemicals/Conditions

• Appropriate storage of hazardous chemicals

• MSDS sheets are readily available on-site.

• Lock out/tag out procedures are in place.

E. Clothing

• PPE is available and properly worn.

• Clothing is relatively dust free with the construction area and patient areas.

WORKMANSHIP: All work under this contract shall conform to the standards of these specifications. The Contractor at no additional cost to the Government shall correct work not meeting these specifications.

Work shall be accomplished by mechanics skilled in the trade.

WORKING CONDITIONS: The Contractor shall take all precautions to ensure that operations are conducted in a manner so as not to interfere with the normal operation of surrounding facilities. The Contractor shall execute work in a safe manner and shall not subject personnel to unsafe conditions.

The contractor will be required to be mindful of the area operations, and patient areas while conducting work or cleaning. The Contractor shall provide safety barricades, devices, and equipment to protect personnel and property and repair damage caused by construction operations. All precautions

7 | P a g e shall be taken to protect the building and its occupants during the period.

While working in building 94 secure patient facility, the contractor must provide a daily check list of tools, equipment and materials to be used. This list must be verified at the end of the day to insure no items are missing. The Contractor should limit the number of materials brought in to only that which is required to complete their immediate task. Likewise, all debris/scraps/waste shall be bagged and removed as soon as the immediate task is completed.

SITE VISIT: The Contractor is urged and expected to visit the site to determine the full extent of the work. Failure to inspect the site will not constitute grounds for a claim after contract award.

DISPOSAL: Disposal of materials removed under this contract at an approved disposal site in accordance with all local, state, and federal laws, regulations, and guidelines are the Contractor’s sole responsibility. The contractor shall adhere to all OSHA requirements concerning removal, transporting, and disposal of the material.

CLEAN-UP: The Contractor shall keep the area neat and tidy and free of debris at the end of each workday. The contractor shall secure all materials and tools outside of the facility daily.

PERMITS: The Contractor shall be responsible for obtaining any licenses and permits and complying with any laws, codes, and regulations applicable to the execution of this work as required Federally and by the State of Texas.

WARRANTY: The contractor shall warranty work to be free of defects and workmanship for the duration of 1 year.
