STATEMENT OF WORK

Renovate B60

17 June 2021

1. PROJECT INFORMATION:

a. PROJECT NUMBER: CZQZ 21-0005

b. PROJECT TITLE: Renovate Building 60

c. PROJECT LOCATION: Building 60 (Base Law Office), Cannon Air Force

Base (AFB), NM

2. DESCRIPTION OF PROJECT:

a. PROJECT BACKGROUND - The Base Law Office (B60) is designed to provide space for the installation staff judge advocate and a courtroom. This facility provides legal advice and support for base personnel for pre-deployment that includes paperwork such as wills, power of attorney, and other legal necessities.

B60 has not been updated in many years. The restrooms are non-ADA compliant, courtroom walls are damaged by rainwater leakage, and the floors squeak, causing hearing difficulty for court reporters. Wires for the audio-visual system present a tripping hazard.

b. CONSTRUCTION SCOPE: The Contractor shall perform all the tasks necessary to deliver the requirements in compliance with this SOW, the Specifications, and Drawings attached (Appendices A & C).

c. Specific items include:

1. This building contains 4 existing restrooms that need to be brought up to code. Per preliminary code review this building needs a minimum total of 3 water closet fixtures and 2 lavatories in the female restrooms, a minimum total of 1 water closet fixtures, 3 urinal fixtures, and 2 lavatories in the male restrooms; 1 mop sink, and 2 water fountains. Provide new walls, doors, toilet partitions, ceramic tile (wall and floor) finishes, paint, ceiling tiles, plumbing fixtures, toilet accessories, light fixtures, and electrical as necessary to reconstruct the existing restrooms to incorporate the increased occupancy in accordance with the UFC 01-200-01 DoD Building Code With Change 1, the 2018 International Building Code, and the Americans with Disabilities Act requirements. Also provide 3M (or equal) opaque film at the existing windows located inside the existing restrooms.

Install new film on the interior side of the building. All new finishes noted above shall meet current base architectural standards (see Appendix B2).

Any new gypsum board installation in the restrooms without ceramic tile should be a level 4 finish. The restrooms currently have ceramic tile on the walls to a certain height with a gypsum board finish above. All new gypsum board installation in restroom areas must be a water-resistant gypsum board.

2. Remove all existing carpet throughout this building. In preparation for new carpet installation: Remove existing vinyl wall base, clean and prep surface. Remove all existing adhesive, clean and prep existing substrate.

Patch/repair/replace any damaged substrate. After installation, provide appropriate transition strips between differing flooring materials and install vinyl base per current base architectural standards (Appendix B2). Refer to Paragraph 9 Hazardous Materials in this SOW for any questionable materials.

3. Remove and replace all existing lay-in ceiling tiles and grid that do not meet current base architectural standards throughout this building. Inspect all areas that currently meet current base architectural standards and replace all damaged tiles throughout the building. Damaged refers to, but is not limited to, water damage, stains, broken, cracked, etc. Repair or replace any damaged lay-in ceiling grid as necessary. Re-seat tiles to lay flat in ceiling grid.

4. In the Courtroom: Remove the existing moveable furniture and store these items onsite in an available temporary location on CAFB in an interior conditioned space for re-use. Carefully remove existing wood chair rail, and save onsite in an available temporary location on CAFB in an interior conditioned space for re-use after new gypsum wall installation. Install new 3/8” gypsum board over existing burlap wall, tape and float, and repaint all walls in the courtroom with a level 4 finish. Replace existing wood chair rail back in original location. Remove and replace all existing carpet and install new vinyl wall base as noted above in paragraph 2.c.2. Remove and replace all existing ceiling structure in this area as noted above in paragraph 2.c.3. Provide 10 additional electrical outlets to accommodate existing audio/visual equipment as indicated on drawings (Appendix A), provide all infrastructure necessary and coordinate final locations with the User (equipment NIC – not in contract). Coordinate with User during construction in the Courtroom to allow for COMM to run CAT 6 cables while walls/floors/ceiling are under construction, and prior to final finishes being installed. COMM installation shall run concurrent to construction. Redesign courtroom per attached drawing (Appendix A) to incorporate changes desired by the User. Remove/replace/renovate existing riser platforms as necessary to achieve new layout, relocate witness stand, extend jury box, and refresh all existing built-in half-walls. Remove existing bead-board and face panels on all half-wall construction of judge stand, witness box, jury box, and prosecution/defense areas, and replace with impact resistant gypsum board, tape and float, provide a level 4 finish, and paint to match.

Repair wood subfloor after removal of existing carpet to reduce noise to an acceptable level in accordance with Chapter 14 of the US Courts Design Guide.

3. GENERAL PROVISIONS :

a. The Contractor shall furnish all services, labor, expertise, materials, supplies, transportation, equipment, and supervision to provide a complete, finished, functional product meeting the provided SOW and Specifications (Appendix C).

b. The contractor must comply with all applicable CAFB standards provided (Appendix B).

4. ENVIRONMENTAL REQUIREMENTS: The Contractor shall comply with all relevant and applicable Federal, State, and local environmental laws, regulations, and executive orders, and base regulations as documented in 27 SOCES Environmental Specification, section 01 57 19 (Appendix C).

5. PERIOD OF PERFORMANCE (PoP): PoP shall be no more than 180 calendar days

6. WORK CONFORMANCE: All work must conform to the current provisions of the CAFB Standards (Appendix B) and the Specifications (Appendix C)

7. SUBMITTALS: Unless otherwise stated, all items are to be in electronic format. Refer to Schedule of Material Submittals for additional required documents, including samples, personnel lists, etc. All submittals are to be provided with an accompanying AF3000 (Appendix D6) cover sheet.

a. SUBMITTAL REGISTER - Provide submittals for all equipment and material according to the Schedule of Materials Submittal (SOMS) throughout project construction PoP, as required by the Specifications Section 01 33 00, Submittal Procedures (Appendix C). Maintain the Register throughout the PoP as required by Specifications section 01 33 00 (Appendix C). CAFB Construction Manager/Contracting Officer’s Representative must review and approve all submittals before commencement of relevant work. The Contractor shall submit the Master Submittal Register to Contracting within 14 calendar days after the Notice to Proceed (NTP).

b. PROJECT SCHEDULE - Provide a detailed project schedule using the AF Form 3064 (appendix D2) in accordance with Specifications section 01 32 01.00 Project Schedule (Appendix C). The contractor shall provide a comprehensive, detailed project schedule in PDF format indicating all major milestones, submittals, critical path items and the anticipated completion date for all construction activities.

c. PROGRESS UPDATES - The Contractor shall submit an AF 3065, Contract Progress Report (Appendix D3) as required by the Specifications section 01 32

01.00. This report shall be signed by the COR’s on-site representative. The work elements and percentages of the total job identified on the AF 3065 shall be identical to the work elements and percentages on the AF 3064. The COR shall obtain the COR acceptance by signature of the actual reported progress prior to submission to the Contracting Officer. The report shall include the following:

1. Current Project Status

2. Expected work activities for the following month

3. Problems and/or concern

d. SAMPLES – Prior to installation the Contractor shall submit color boards (for paint), color samples of carpet and tile, samples of cabinets (wood color), samples of counter tops (color/pattern), etc. Each sample shall be approved prior to installation. Refer to Schedule of Material Submittals for more information.

e. INVOICES - Each invoice shall be accompanied by an AF 3065 signed by the CM/COR. The AF 3065 and invoice shall all cover the same time period

f. WORK CLEARANCE REQUEST - The contractor shall submit the most current CAFB Form 103 (Appendix D4) Work Clearance Request prior to starting any facility work or related utilities work for the project. Meetings for Work Clearance Requests are held in Bldg. 355 on Thursday at 7:30 AM. The meetings for Work Clearance are not mandatory and take place post-award of project.

g. DD FORM 1354 – For all projects, the Contractor shall prepare and submit a DRAFT DD Form 1354 (Appendix D1), Transfer and Acceptance of Military Real Property, Current Version. This DRAFT DD Form 1354 establishes the level of detail and the different components of a project for all work accomplished under the project, including installed property, by building/facility number, and shall be submitted to 27th Special Operations Contracting Squadron. The DRAFT DD Form 1354 shall be submitted no later than 30 calendar days after the Notice to Proceed (NTP). This DRAFT DD Form 1354 (Appendix D1) shall be reviewed by Real

Property Office (RPO) to resolve any issues and provide clarity to the Contractor for the inclusion in the INTERIM DD Form 1354. Refer to attached sample DD Form 1354 (Appendix D1).

h. UTILITY OUTAGE REQUEST - The contractor shall submit the most current CAFB Form IMT 3624 (Appendix D5) for utility outages prior to starting any utilities outages for the project. Meetings for Utility Outage Coordination are held in Bldg.

355 on Tuesday at 7:30 AM. These meetings are not mandatory and take place post-award of project. Contractor should provide 21 calendar days’ notice for any utility outages.

i. TRAFFIC CONTROL - If the contractor requires parking lot closures, a traffic control plan (see Specifications section 01 50 00 Temporary Construction Facilities and Controls (Appendix C) must be submitted for approval to the CO at least 14 calendar days in advance.

8. BASIS OF DESIGN (BOD): Any brand name items listed in this SOW and drawings are intended as a BOD only.

a. The Contractor may substitute a product, or products, for the brand name items as long as the substituted product(s) meet the minimum performance standards established by the listed salient characteristics. Refer to Specification Section 01 00 00 GENERAL REQUIREMENTS (Appendix C). An ‘Or Equal’ product shall be submitted with the initial proposal and will be reviewed for technical acceptability.

b. The Contractor is responsible for how the substituted product(s) affect the design and schedule of the project. 27 SOCES shall review the substituted products for general conformance only. All substituted products will be reviewed for compliance with the Specifications, any SHPO or historical considerations, Environmental regulation compliance and any other review processes that are applicable to this project.

c. As a part of the 27 SOCES review process for substituted products, and prior to the substituted product being used or installed, the Contractor must demonstrate in writing that the substitution will meet all project requirements listed above.

9. HAZARDOUS MATERIALS: The structure has previously been confirmed to contain asbestos; no material containing in excess of 1% asbestos has been confirmed to be currently present. The Contractor shall take care to ensure any suspicious material is not rendered friable during the course of the work. Any suspicious material discovered should be reported to the Construction Manager and Contracting Officer.

10. QUALIFIED PERSONNEL: The Contractor shall ensure that only qualified personnel carry out the tasks outlined in Specification Section 01 35 26 para 1.6.1.2 (Appendix C).

Qualified is defined as registered professional or, where registration is not applicable, trained and/or certified in their respective field. The Contractor shall have 14 calendar days to replace key personnel on the project: Project Manager, Site Superintendent, Site Safety and Health Officer, and the Quality Control Manager.

11. CONSTRUCTION REQUIREMENT: The project shall conform to all applicable Federal, State and local laws, regulations and base-specific standards and specifications. All work accomplished by the Contractor shall comply with these criteria. Should a deviation be required, the contractor shall request, and receive written approval from the CO.

12. SECURITY: The Contractor shall provide personnel who meet security requirements and can gain and maintain access to the base for the duration of the project. The Contractor shall provide Entry Access List (EAL) in accordance with (IAW) current CAFB policy.

13. ADMINISTRATIVE REQUIREMENTS:

a. The Contractor shall identify a lead Program Manager who shall perform all program management functions and tasks necessary to satisfy the requirements consistent with operational quality, safety, security, and environmental standards as required in Specification Sections 01 30 00 Administrative Requirements, 01 35 26 Governmental Safety Requirements and 01 45 00.10 20 Quality Control For Minor Construction (Appendix C). The program management function of this contract shall provide insight to the Government for all technical and programmatic activities performed under this contract.

b. ON-SITE SUPERVISION – The Contractor shall provide the manpower, equipment, materials, services, and transportation necessary to ensure oversight services throughout accomplishment of this contract. It is the responsibility of the Contractor to establish and maintain open communications between salient authorities associated with accomplishment of this contract and as required in Specification Sections 01 30 00 Administrative Requirements, 01 35 26 Government Safety Requirements, and 01 45 00.10 20 Quality Control for Minor Construction (Appendix C).

14. WORK PLANNING/COORDINATION:

a. CONTRACTOR OPERATIONAL HOURS – The Contractor shall establish a work schedule which requires work to be performed from 7:00 AM to 4:00 PM, Monday through Friday. The Contractor shall provide written request for additional hours (overtime) to the CO a minimum of 14 days prior to the requested time interval.

The contractor shall receive written approval from the CO prior to any overtime being worked. Refer to Specification Section 01 01 00 General Requirements (Appendix C) for additional information. Coordinate work schedules with the 27 SOCES CM at least seven days in advance.

b. STAGING AREA – The Government shall identify space on the jobsite or in a base Contractor Staging Area where the Contractor may deliver and store non-hazardous materials within the installation.

c. VERIFICATION OF EXISTING CONDITIONS & SITE SURVEY – Use of existing condition data provided by the USAF conveys acceptance and as such does not relieve the Contractor of liability associated with performance as the Engineer of Record. The Contractor shall be solely responsible for verification and validation of existing conditions, coordination of existing conditions in parallel with proposed requirements, and above and below-grade condition assessments. The Contractor shall visit the premises and take into consideration all conditions which might affect the work.

d. PRE-CONSTRUCTION SITE SURVEY – Prior to the start of any on-site construction activities, the Contractor shall:

1. Conduct a project site condition survey with the 27 SOCES CM.

2. The Contractor shall note the condition of trees, shrubs, grassed areas, concrete, and any structural damage that may exist immediately within or adjacent to the work site. Existing infrastructure that is not part of the project shall be maintained in working order. If damage occurs, it is the responsibility of the Contractor to restore the infrastructure to equal or better condition than it was prior to the damage. If the Contractor fails to note conditions, the Government shall direct the Contractor’s repairs and may require a full removal and replacement of the damaged infrastructure by qualified personnel.

3. The Contractor shall field verify all dimensions or examine covered connections before ordering materials or performing any work.

e. PROJECT MEETINGS – The Contractor shall at a minimum hold meetings as required in Specification Section 01 30 00 Administrative Requirements (Appendix C) and as noted below:

1. Pre-Construction Meeting: A pre-construction meeting shall be held at the 27 SOCONS conference rooms or via teleconferencing within 14 calendar days after contract award.

2. Construction Meetings: Construction meetings will be held at the contractor’s jobsite trailer or by teleconference. The Contractor shall be responsible for providing meeting minutes. Date and time shall be set by the COR at the pre-construction meeting.

3. Red Zone Meeting: Red Zone meeting shall be held at 80% project completion or within 45 calendar days of Beneficial Occupancy, whichever comes first.

15. DELIVERABLES:

a. Unless otherwise stated, all deliverables shall be provided electronically.

Deliverables shall be formatted for use on the latest version and compatible with Microsoft Word, Excel, Adobe Acrobat and, if applicable, AutoCAD. All deliverables shall be dated. Samples may be provided to the Contractor upon request. Refer to Specifications Section 01 33 00 Submittal Procedures (Appendix C) for additional submittal information.

b. DESIGN AND SPECIFICATIONS: The Government will provide the Contractor access to the 35% design drawings and specifications including any available existing engineering plans, as-built drawings, diagrams, aerial photographs, digitized map files, etc. to facilitate evaluation of the site/building, as needed. Use of any such available existing engineering plans, as-built drawings, diagrams and other data provided by the USAF does not relieve the Contractor of his/her responsibility for field measurement, verification and validation of existing conditions in the effort of developing their own necessary design/construction drawings for this project.

c. SCOPE OF WORK: Work shall conform to the requirements outlined by the design drawings, specifications and this SOW. All materials and equipment installed shall be new. Reuse of materials and equipment is not approved. The contractor shall provide all labor, materials, tools, equipment, paperwork, etc. required to furnish, install and accomplish the following:

1. The Contractor shall accomplish the following pre-construction actions:

a. Pre-construction site visit/survey

1. Note the condition of existing infrastructure that may be used, traversed, entered/exited or otherwise impacted by the project.

b. Field verify existing conditions and dimensions

1. Utility detection/identification

2. Permitting/work approval

3. Submit and receive approval of required submittals according to the submittal register, specifications, SOMS and this SOW.

2. The Contractor shall accomplish the demolition as indicated by the Government provided drawings.

3. The Contractor shall accomplish the construction/installation as indicated by the Government provided drawings.

4. Prior to project closeout the Contractor shall accomplish the following clean up:

a. Sweep/vacuum debris resultant from project waste.

b. Remove and dispose of all resultant project waste off-site.

c. Removal of all materials/equipment from laydown yard/storage area.

d. Change all filters in all HVAC units.

d. CLOSEOUT DOCUMENTS – The Contractor shall provide all documents associated with project closeout per Specification section 01 78 00 Closeout Submittals (Appendix C).

e. GOVERNMENT REVIEW – For all submittals and deliverables requiring Government review, the Contractor shall allow 14 calendar days for Government processing and review.

16. PROJECT CLOSEOUT:

a. The project closeout documents required below shall be submitted to and accepted by the CO prior to final payment.

b. GENERAL REQUIREMENTS – The Contractor shall perform all closeout requirements as shown in Specification Section 01 78 00 Closeout Submittals and 01 78 23 Operations and Maintenance Data (Appendix C).

c. PRE-FINAL/FINAL INSPECTION - The Contractor shall conduct a Pre-Final Inspection and Final Inspection with project associated CAFB personnel (PM, COR, & CA at a minimum) and publish the findings in a Pre-Final Inspection Report. The Contractor shall publish the Pre-Final Inspection findings in a Pre- Final Inspection (Punch List) Report. All punch list items shall be resolved prior to acceptance. At the Final Inspection, the Contractor shall present a completed DD Form 1354 to the 27 SOCES CM for signature and acceptance by the Base Civil Engineer (BCE). Refer to Specifications Section 01 78 00 Closeout Submittals for additional closeout information (Appendix C).

d. AS-BUILT DRAWINGS – The contractor shall provide as-built drawings using the Computer Aided Design (CAD) and Geographic Information System (GIS) guidelines provided by the Government. The construction contractor shall ensure their as-built CAD and GIS submittal is compliant with the latest CAFB CAD/GIS standards (Appendix B6).

e. GEOGRAPHIC INFORMATION SYSTEMS (GIS) DATA – All new facilities and utilities shall be located by the Contractor in an electronic format that is compatible with the current base GIS software and includes descriptive information required by the USAF. In such cases, the Contractor is required to assist USAF agents or personnel in the generation of GIS information if requested by the 27 SOCES PM.

Subject to change, the following are the basic requirements for GIS information to be delivered to the Air Force electronically:

1. DATUM – WGS84

2. Coordinate System – UTM

3. Units – Meters

4. Data in attribute Table will be SDSFIE version 2.6 compliant

5. Shape file Geodatabase format

f. OPERATIONS & MAINTENANCE (O&M) DATA – The Contractor shall submit one hard copy and one electronic copy containing operations and maintenance information prior to project closeout, per Specifications Section 01 78 00 Closeout Submittals (Appendix C). The O&M data shall be submitted to the Government within seven calendar days of final inspection.

g. WARRANTY INFORMATION - The Contractor shall provide a one year warranty on labor and materials in addition to manufacturer warranties. The warranty shall be issued in accordance with FAR 52.246-21. The warranty data shall be submitted to the Government within seven calendar days of final inspection. The contractor shall also provide documentation on all manufacturer’s extended warranties, per Specifications Section 01 78 00 Closeout Submittals (Appendix C).

h. REAL PROPERTY ACCEPTANCE – At 80% of project completion or no later than 60 days prior to total asset placed-in-service date or partial asset placed-in service date, submit an INTERIM DD Form 1354 (per project number) to the CO for Real Property review. At Total or Partial Placed-In-Service-Date (PISD, also referred to as BOD), the closeout package must include INTERIM DD Form 1354 with list of all Real Property Installed Equipment (RPIE). Any deficiencies and/or punch list items identified during the pre-final/final inspection must be included in Box # 27 or attached to the INTERIM DD Form 1354 (Appendix D1). At project completion, the following items shall be delivered to 27 SOCONS:

1. Final INTERIM DD Form 1354 (Appendix D1)

2. As-Built Drawings

3. O&M manuals in both hardcopy and electronic format.

i. FINAL PAYMENT – Prior to final payment, the contractor shall submit and receive approval of all required submittals and complete all work according to contract documents and written agreements.
