About this file

This is a Past Performance Reference form (Attachment 02) used for federal contract solicitations, specifically identified as solicitation N6426726R1030.

The form is a template designed to collect historical contract performance data from offerors in response to solicitation requirements. It requires contractors to complete two primary sections for each relevant past contract or task order: (1) Past Performance Data, which captures identifying information including the performing contractor/division, CAGE Code/DUNS number, customer/program name, contract number, period of performance, and customer points of contact (Program Manager, Contracting Officer, Administrative Contracting Officer, COTR/COR, and Task Monitor); and (2) Contract Information, which documents contract type (Fixed-Price, Firm-Fixed-Price, Labor Hour, Time-and-Materials, or Cost-Reimbursement variants), award dollar amounts, completion dollar amounts, current obligated costs, and estimated costs to complete for ongoing contracts. The form also requires a Summary Overview of Corporate Experience section and narrative responses addressing: (a) how the past contract scope relates to the current solicitation in size and scope; (b) significant achievements, challenges, or obstacles encountered and mitigation measures; (c) specific performance criteria/measures with target and achieved performance levels for the most recent period; and (d) for Large Business Offerors, documentation of small business subcontracting goal performance, including specific goals, actual progress, and corrective actions when goals were not met, with additional requirements for those with other SeaPort-NxG Task Orders to report both Task Order and MAC-level goal performance.
