About this file
This is a Statement of Objectives (SOO) for the procurement of three Gen AI-powered patient simulator systems for the 312 TRS Fire Academy at Goodfellow Air Force Base, Texas. The system, designated as the ALEX Gen AI platform, must integrate advanced Generative Artificial Intelligence communication capabilities with comprehensive physical assessment features to provide immersive, adaptive training for healthcare personnel in both patient communication and hands-on clinical interventions.
The system must deliver the following core capabilities: AI-powered patient conversations that respond dynamically to learner questions with natural conversational speech interaction and adaptive responses that personalize each simulation experience; scenario management features allowing instructors to automatically generate new scenarios using AI text prompts and access a comprehensive scenario library aligned with core healthcare competencies; and an automated, AI-driven evaluation system for objective learner assessment and after-action reviews. Clinical assessment capabilities must include visible chest rise and fall synchronized with physiological status, comprehensive airway management including intubation and Bag-Valve-Mask ventilation, active CPR performance tracking with integrated defibrillation feedback, bilateral pulse simulation at a minimum of four distinct anatomical points, realistic heart, lung, and bowel auscultation sounds, and SmartCuff or equivalent blood pressure measurement capabilities. The system must operate in compliance with Department of War unclassified data handling protocols, with the Gen AI model prohibited from ingesting, processing, or outputting real Protected Health Information or Personally Identifiable Information; all training scenarios, generated dialogue, and patient profiles must be exclusively synthetic. Delivery must be coordinated at least 48 hours in advance with Jesse A Wagner, GS-11, at 312 TRS Fire Academy, 301 Comanche Trail, Building 3453, Goodfellow AFB, TX 76908, by phone at 325-654-1186 or email at Jesse.Wagner.6@us.af.mi.

Statement of Objectives (SOO) Gen AI Patient Simulator x3
Goodfellow AFB, TX
1. Purpose
The purpose of this acquisition is to procure a next-generation patient simulator that seamlessly integrates advanced Generative Artificial Intelligence (Gen AI) communication capabilities with comprehensive physical assessment features. This system will provide immersive, adaptive training to improve healthcare personnel competency in both patient communication and hands-on clinical interventions, ultimately enhancing medical readiness.
2. Scope
The contractor shall provide a complete Gen AI-powered patient simulator system (ALEX Gen AI platform) that combines communication training with hands-on patient assessment skills. The system must support natural conversational speech, dynamically generated scenarios, AI-driven evaluations, and a full suite of physical clinical simulation capabilities.
3. System Objectives & Capabilities
The simulator must achieve the following functional and physical objectives:
Functional Area Objective / Capability Requirement
AI & Conversational Training
AI & Conversational Training
AI & Conversational Training
Implement Gen AI-powered patient conversations that respond dynamically to learner questions.
Enable natural conversational speech interaction to simulate realistic patient dialogue.
Provide adaptive patient responses that personalize and alter each simulation experience based on learner interaction.
Scenario Management Allow instructors to automatically generate new simulation scenarios using simple AI text prompts.
Scenario Management Provide a comprehensive scenario library that is strictly aligned with core healthcare competencies.
Learner Assessment Deliver an automated, AI-driven evaluation system to support objective learner assessment and after-action reviews.

Clinical Assessment:
Diagnostics
Feature visible chest rise and fall synchronized with the simulated patient's physiological status.
Support comprehensive airway management, including intubation procedures and Bag-Valve-Mask (BVM) ventilation.
Provide active CPR performance tracking and integrated defibrillation feedback.
Simulate bilateral pulses accurately at a minimum of four distinct anatomical points for circulation checks.
Replicate realistic heart, lung, and bowel auscultation sounds for diagnostic training.
Clinical Assessment: Vitals Include SmartCuff (or equivalent) blood pressure measurement capabilities.
4. Operating Constraints & Security Protocols
The system must operate in compliance with Department of War (DoW), unclassified data handling protocols. Gen AI model utilized must not ingest, process, or output real Protected Health Information (PHI) or Personally Identifiable Information (PII). All training scenarios, generated dialogue, and patient profiles must be exclusively synthetic to ensure strict privacy compliance.
