About this file

This document contains a past performance questionnaire template to be used for evaluation in awarding a federal contract. It requests information about a contractor's performance on past and current contracts within the last three years, to be completed by both the contractor and a point of contact. Questions address compliance with requirements, project management effectiveness, timeliness, cost control, and commitment to customer satisfaction. The template also provides for comments and identifies other sources of past performance information.

This federal contract opportunity is a five-year Small Business Set-Aside IDIQ for Wireless AT Platform Test Adapter Kits. The Army Materiel Command Contracting Command at Redstone Arsenal is soliciting proposals due by June 3, 2021. The solicitation includes statements of work, data requirement lists, and pricing spreadsheets for spare parts in out years. This amendment provides additional details for a wireless test set procurement.
