About this file
This is a Past Performance Questionnaire for solicitation W912CH-25-R-0176 regarding the procurement of collapsible fabric tanks. The questionnaire is designed to be completed by previous or current customers of potential offerors to assess contractor performance on relevant past contracts involving 3,000-gallon, 20,000-gallon, 50,000-gallon, and 210,000-gallon collapsible fabric tanks.
The questionnaire evaluates contractors across four primary performance categories: (1) Quality, assessing workmanship compliance with government technical data packages, specifications, testing requirements, quality assurance processes, and Factory Acceptance Test (FAT) approval achievement; (2) Project Schedule, evaluating adherence to original delivery schedules and timeliness of product deliverables; (3) Management/Business Relations, including program management responsiveness, problem identification, corrective action implementation, and subcontract management; and (4) General Comments for any additional relevant performance information. Respondents rate performance using a six-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Too Soon to Tell) and provide detailed narrative comments explaining ratings. For adverse performance issues, respondents must assess the extent of contractor fault versus government/customer fault or factors beyond the contractor's control. Completed questionnaires must be submitted to Lyndsey Pena (lyndsey.m.pena.civ@army.mil) and Katherine MacFarland (katherine.e.macfarland.civ@army.mil) by the proposal submission deadline.
