About this file

This is a Past Performance Matrix template that offerors must complete as part of their proposal submission for RFP W912CH-25-R-0176. The template is designed to document and evaluate a contractor's relevant past performance on similar contracts performed within five years of the RFP issuance date.

Offerors are required to populate the matrix with specific contract information in designated yellow-highlighted cells, including contract number, prime contractor name and address, CAGE code, contract type, period of performance dates, delivery schedule/rate, contract value range, and points of contact from both the government or private industry side. The template specifies that only contracts performed by the offeror as the prime contractor qualify, with limited exceptions for associated corporate entities, predecessor companies, or joint venture partners as defined in Section L.4.1. Offerors must identify the government or private industry contracting activity and provide the name, phone, and email of the PCO or contracting POC, along with documentation of when the past performance questionnaire was sent to references. The instructions explicitly prohibit manipulation of the spreadsheet beyond filling in requested information and embedding attachments. The template serves as a critical evaluation tool for assessing contractor capability and experience relevant to this Army solicitation for collapsible fabric tanks under an IDIQ contract vehicle.

1 SUBMISSION1 SUBMISSION
Contract Number
(including Task/Delivery Order number if applicable)1) It is incumbent upon the offeror to ensure that a submission meets the definition Contract Submission requirement defined in Section L.4.1. When applicable, only propose a single Order (such as a task/delivery order, work directive, or technical instruction) if the Order is part of broader contractual instrument such as an Indefinite Delivery Contract, BOA, BPA, FSS. Broad ordering instruments such as Indefinite Delivery Contracts (e.g. IDIQ), BOA), BPA, and FS) typically DO NOT demonstrate actual performance, and as such, would not meet the definition of a Contract. If a contract instrument executed under a broad ordering instrument (such as a task or delivery order, etc.) also functions as a broad ordering instrument and does not require or demonstrate that a distinct effort be performed (i.e. requires other written instruments to execute performance), then this contract instrument would NOT meet the definition of a Contract.
