About this file

This is a Subcontractor/Teaming Partner Consent Form template for the Repair HICS Gates solicitation (FA4626-26-R-0010) at Malmstrom Air Force Base in Montana. The form is designed to obtain written consent from major subcontractors and teaming partners to allow the Government to disclose their past and present performance information to the prime contractor during the source selection evaluation process. Prime contractors are required to complete the bolded sections of the template with their company name and details before distributing it to their respective subcontractors and teaming partners for execution. The completed consent forms, signed by individuals with legal authority to bind their companies, must be submitted as part of Volume I - Past Performance documentation in the proposal.

This attachment is a procedural document supporting the Government's past performance evaluation methodology for the 5-year Requirements contract for HICS gate repair and replacement services. It does not contain specific technical requirements, pricing terms, or performance metrics, but rather serves as an administrative tool to facilitate the evaluation process under the 100% Service-Disabled Veteran-Owned Small Business (SDVOSB) set-aside requirement.

FA4626-26-R-0010

HICS Gates Project No. NZAS 25-6010

SUBCONTRACTOR/TEAMING PARTNER CONSENT FORM FOR THE RELEASE OF PAST AND PRESENT PERFORMANCE INFORMATION TO THE PRIME CONTRACTOR Past performance information concerning subcontractors and teaming partners cannot be disclosed to a private party without the subcontractor's or teaming partner's consent. Because a prime contractor is a private party, the Government will need that consent before disclosing subcontractor/teaming partner past and present performance information to the prime during exchanges. To assist the Government's Evaluation Team in assessing your past performance relevancy and confidence, we request that the following consent form be completed by the major subcontractors/teaming partners identified in your proposal. The completed consent forms should be submitted as part of your Volume I – Past Performance. The following consent form requires editing by the prime contractor prior to forwarding to their respective subcontractors (see bolded text).

Ms. Elizabeth Basu / Ms. Colleen Goad, We are currently participating as a (subcontractor/teaming partner) with (prime contractor or name of entity providing proposal) in responding to the Department of the Air Force, Malmstrom AFB Request for Proposal FA4626-26-R-0010 for the HICS Gates project for Malmstrom AFB, MT.

We understand that the Government is placing increased emphasis on past performance to obtain best value in source selections. To facilitate the performance confidence assessment process, we are signing this consent form to allow you to discuss our past and present performance information with the prime contractor during the source selection process.
