About this file

This is a Past Performance Reference Sheet template for offerors to complete and submit as part of their proposal to solicitation FA462626R0010 for Repair HICS Gates at Malmstrom Air Force Base. Offerors must provide between three and five recent and relevant past performance references within Volume I of their proposal, with submissions due no later than September 3, 2026, at 2:00 p.m. Mountain Standard Time. "Recent" is defined as projects completed or ongoing within five years of solicitation release, and "relevant" is defined as projects with similar scope, size, and complexity valued at $10,000 or greater. For each reference, offerors must document the contract/task order number, performance dates, contract dollar value, customer name and point of contact information, whether the contract was competitively awarded, the contractor's role (prime, subcontractor, or key personnel), all teaming arrangements, and a detailed description of work performed.

The template includes a Performance Information section requiring offerors or customer references to rate the contractor's performance across 24 evaluation criteria using a five-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable). Evaluation areas assess management and staffing capabilities, quality control procedures, problem identification and resolution, schedule adherence, responsiveness to changes, financial management, cost proposal accuracy, small business subcontracting goals, and overall performance suitability for contract award. Supporting information must be provided for each rating, and customer references must indicate whether a cure or show cause notice was issued and whether they would award another contract to the offeror. Offerors may alternatively submit existing CPARS (Contractor Performance Assessment Reporting System) reports in lieu of completing this form.

Past Performance Reference Sheet In accordance with (IAW) paragraph 4 of Section L of this solicitation, please provide between three (3) and five (5) recent and relevant past performance references. As a reminder, “recent” is defined as “completed or ongoing projects within the last five (5) years from the date of the release of this solicitation”, and “relevant” is defined as “a project with a scope similar in scope, size, and complexity to the requirements of the solicitation and valued greater than or equal to $10,000.00.” Include rationale supporting your assertion of relevance.

One of the considerations in proposal evaluation is the verification of the offerors past performance on contracts or other work efforts, which reflect the offeror's ability to perform on the proposed effort. We depend on information received from organizations such as yours, which have had first-hand experience with the offeror, for the evaluation of the offeror's performance on those contracts or work efforts.

Past performance references must be provided within Volume I – Past Performance of your company’s proposal, which is to be submitted and received no later than 03 September 2026 at 2:00 p.m. Mountain Standard Time (MST).
