Performance Assessment Questionnaire – Cover Letter
SUBJECT: Offeror Past Performance Assessment in Support of Request for Proposal (RFP) W9127N25R0011 – Lower Columbia River Armed Guard Services Good day, The US Army Corps of Engineers, Portland District is currently conducting a competitive source selection to evaluate offerors on the subject RFP. As part of this evaluation, we have requested that the offerors provide information about their past performance and experience on recent and relevant federal, state, local government or commercial contracts for armed guard services, and describe the offeror’s experience to performing a contract the same or as similar to the RFP requirements. The type of service is for roving foot and vehicular patrols, building security and CCTV monitoring, area sweeps, guard sentry/visitor and gate control, with badging along with other services. A full description is posted on www.sam.gov. The previous contract was approximately $12,000,000 over 5 years at three federal facilities.
Interviews are anticipated based on available time. We may also email a fill out sheet. Please share your insights to help us evaluate the offeror’s past performance record and experience with performing the kind of work in the RFP. If a written reply, please complete the enclosure and return to the Contract Specialist Nathean Stoner at nathean.w.stoner@usace.army.mil as soon as possible, with up to 3 business days being the target date from receipt. We will first try and contact you to get this information so we can answer any questions at that time. Please mark the email reply with an indicator that the content contains past performance information, so we may safeguard the content appropriately.

Performance Assessment Questionnaire
Please provide your candid responses. The information that you provide will be used in the awarding of federal contracts. Therefore, it is important that your information be as factual, accurate and complete as possible to preclude the need for follow-up by the evaluators. Rating Definitions:
Substantial Confidence: Performance meets contractual requirements and exceeds many requirements that benefit the end user. Work was accomplished with few, if any, minor problems for which corrective actions taken by the contractor were highly effective. The offeror has been highly successfully in performing the required effort.
Satisfactory Confidence: Performance meets contractual requirements and exceeds some requirements that benefit the end user. Work was accomplished with some minor problems for which corrective actions taken by the contractor were effective. The offeror has successfully performed the required effort.
Limited Confidence: Performance does not meet some contractual requirements. Serious problems with contractor performance were experienced for which the contractor has either not yet identified corrective actions or the corrective actions taken appear only marginally effective. The offeror has had little success performing the required effort.
No Confidence: Performance does not meet most contractual requirements. Serious problems with contractor performance were experienced for which the corrective actions were ineffective. The offeror has not successfully performed the required effort.
