COMMANDER’S POLICY MEMORANDUM #7

SUBJECT: Portland District Commander’s Critical Information Requirements (CCIR)

1. Reference.

NWD Daily Tasking Order 24-12-09 (NWD Commander’s Critical Information Requirements)

2. Purpose. To prescribe CCIR roles and responsibilities within the Portland District, ensuring situational awareness in critical situations.

3. Applicability. This Commander’s Policy Memorandum applies to all reportable situations outlined in the enclosures.

4. Methods of communication. NWP will use two linked methods of communication for CCIRs: command channels and the official reporting system of record, ENGLink.

a. Command channel communication is the first requirement and conducted via email (or followed up by an email after telephonic communications). All NWP CCIRs require email notification from Division Chief to the NWP Commander (DE), Deputy Commander (DD), Deputy District Engineer for Project Management (DPM), and Chief of Staff (CoS). Leaders sending reports should include other Division and staff points of contact as appropriate (see Enclosure 1), NWP CCIR Matrix for recommended internal distribution).

b. The NWP Emergency Operations Center (EOC) formally reports CCIRs on the

ENGLink Emergency Management (EM) Dashboard utilizing the information provided to NWD in the NWP command notification. Upon completion, EOC will click the “Email a link” button creating an email that will be sent to the CENWD-EOC mailbox at CENWD- EOC@usace.army.mil. In addition to the link, a brief synopsis of the event will be included. CCIRs 7, 11, and 38 will be posted by the Local Safety Office on the ENGLink Safety Dashboard (separate and does not communicate with the EM Dashboard).

c. With exception of sensitive information (e.g. Sexual Assault Response

Coordinator (SARC)), leadership will courtesy copy any appropriate NWD/NWP EOCs and other interested offices (see Encl 1 for details). NWP EOC may be contacted via email (CENWP-EOC@usace.army.mil) and phone (503-808-4402).

d. These reports are releasable under FOIA and discoverable in litigation, on account of which care must be taken relative to PII, CUI, and other information whose public release might violate DOD/DA/USACE guidance or be adverse to USACE interests in legal or administrative proceedings.

5. Reporting Format. “4 WINs” refers to a method of information collection: Who, What, Where, When, Impacts, and Next Report. When reporting, present all critical information. For a simple example - see below:

CAT 3 CCIR #23

Who: Northwestern Portland District (NWP), The Dalles Dam What: Observed unlawful operations of Unmanned Aircraft System (UAS) on NWP project lands.

Where: Vicinity of The Dalles Dam and The Dalles City limits.

When: 16 1450 U OCT 2021, duration 40 minutes.

Impacts: Security Implications that could affect The Dalles Dam and personnel.

Next Report; We will continue to monitor and produce a Be On the Look Out (BOLO) report for the UAS, next report 17 1500 U OCT 2021.

6. CCIR Categories. There are five categories of CCIRs (CAT1, CAT2, CAT3, CAT4, and CAT5), each based on the criticality of the information. Each category has different distribution requirements. It is important to use the correct CAT and CCIR number when reporting because it is a recommendation to the command as to the level of reporting.

a. CAT 1. The most critical events are designated CAT 1 CCIRs. When a CAT 1 event occurs, the NWP Commander (or designee) must immediately contact the NWD Commander (or designee) who will then notify the Chief of Engineers through the DLL- HQ-CCIR-CAT1 distribution (and telephonically, if command deems it appropriate).

(1) Due to the criticality of CAT 1 CCIRs, NWP will provide a CCIR report through command channels and post reports in ENGLink. NWD will release the CCIR to HQ via ENGLink after the Chief has been notified.

(2) In addition to HQUSACE directed CAT 1 CCIRs this policy memo provides additional reporting requirements for the following NWD/NWP events.

(3) CCIR #12 – Mission Degradation. Event of circumstance causing significant operational detriment and inability to perform critical NWP/NWD Missions. NWP will post these reports via ENGLink and will post a Project Status Report (PSR) via the UCOP as well.

(4) CCIR #8 – Cost or schedule increase on a USACE project that exceeds project contingency/float. Districts will report using the formal reporting tool submitted in Project Management Information System (PROMIS) to UOC for further dissemination.

(5) CCIR #4 – Reports of sexual harassment and sexual allegations against Commanders (O5 or above CDR) and SARC’s as well as those where there are numerous reports of sexual assault from a single unit, and those likely to draw high media attention is likely. MSC Commander will send CCIR encrypted to USACE Commanding General (CG), Deputy Commanding General (DCG), and Commanding Sergeant Major (CSM); copy to USACE Sexual Harassment Assault Response and Prevention (SHARP) Program Manager (PM), Division SARC, and Chief of Staff (CoS).

No ENGLink reporting.

b. CAT 2. CAT 2 CCIRs require the Division Commander (or designee) to immediately notify the DCG through DLL-HQ-CCIR-CAT2 distribution. NWP EOC will enter the report in to ENGLink, and forward to NWD. NWD will release the CCIR to HQ via ENGLink after the Chief or DCG has been notified.

c. CAT 3. CAT 3 CCIRs are reportable to UOC only via ENGLink and via specific email (DLL-HQ-CCIR-CAT3) distribution.

(1) At first light, the District EOC will email the NWD EOC (phone as needed for clarity), and the NWD EOC will them email the UOC.

d. CAT 4. These are the NWD Commander’s designated CCIRs that do not require further reporting to HQUSACE and do not require posting in ENGLink.

(1) District will report CAT 4 CCIRs via email through both command channels and thru EOC channels.

(2) Note the NWD CCIR #37 is a modified version of HQUSACE CCIR #10 (Navigation Areas of Concern), used for reporting a restricted navigation channel (less than 9-ft depth & less than 300-ft width – but remains passable; vessels may need to double trip or light load).

e. CAT 5. These are NWP Commander’s designated CCIRs that do not require further reporting but may be sent upward by the Commander at his or her discretion.

(1) #35 Any High Hazard Mishaps, which includes any Near miss reports must be submitted to the ENGLink Safety Dashboard within 7 days.

(a) Electrical – to include Arc Flash, electrical shock, etc.

(b) Uncontrolled release of Hazardous Energy (includes electrical and non-electrical);

(c) Load Handling Equipment (LHE) or Rigging.

(d) Fall-from-Height- (any level other than same surface), and

(e) Underwater Diving.

(2) #36 Mission Degradation. Event or circumstance causing operational detriment to NWP mission or project, but not significant (CCIR 12) at time of reporting.

(a) 36a. Critical Plant: Non-mission capable or degraded performance of critical infrastructure (i.e., dredges, inoperable spillway gates, stilling basins, levees, dam structures) extending past 24 hours.

(b) 36b. Power: Unscheduled unit outage causing loss of generation lasting longer than 12 hours (i.e. mechanical failure, turbine, generator, or transformer out of service).

Transmission faults resulting in generation disruption.

(3) #37 Environmental: Significant environmental event (i.e., reportable hazmat spill, any oil release in a waterway, endangered species takes, and fish kills).

(4) #38 Security Incidents: theft, vandalism and break-ins to district utilized facilities and equipment.

(5) #39 SPOTREP: For items that don’t fall within in a specific category but are deemed to be significant enough to inform the NWP commander.

7. Enclosure 1 provides a complete list of NWP, NWD, and HQUSACE CCIR reporting charts. This list is not all-inclusive but should allow a reasonable person to evaluate the need and timing on notifying the Command. Enclosure 2 is NWD Daily Tasking Order 24-12-09 (NWD Commander’s Critical Information Requirements).

9. Supersession. This policy supersedes Commander’s Policy #7, dated 10 March 2023.
