About this file
This is a Past Performance Questionnaire template designed to evaluate contractor performance for the Pojoaque Basin Regional Water System Phase II Design-Build contract opportunity. The questionnaire is to be completed by references identified by offerors and returned to Bureau of Reclamation Contract Specialist Mary B. Maestas at mary_maestas@ios.doi.gov no later than 3:00 p.m. MDT on May 12, 2026.
The questionnaire uses a five-point rating scale (Outstanding, Better, Acceptable, Marginal, and Unacceptable) to assess contractor performance across multiple dimensions. References are required to provide detailed explanations for each rating, with particular emphasis on adverse ratings (Marginal or Unacceptable), which must include specific events and explanations of corrective actions per FAR 15.306 requirements. The evaluation criteria are organized into five categories: Contract Information (including contractor name, agency/client details, contract type, award amount, performance period, and subcontractor information); Technical Rating (staff qualifications, experience, and data collection/analysis); Management Rating (project management, schedule adherence, scope management, resource allocation, internal communication, and billing accuracy); Quality Rating (product/service quality and problem resolution); and Customer Satisfaction Rating (communication, customer service, and professional conduct). The template also includes general questions regarding contractor strengths, weaknesses, proposal alignment, and likelihood of future engagement.
