About this file

This is a Past Performance Questionnaire template for the Missile Defense Agency (MDA) contract HQ085426RE004 Advanced Planning Services (APS). The questionnaire is used to collect past performance information from government assessors regarding contractor performance on previous relevant contracts to support source selection decisions.

The form is divided into six sections completed by different parties. Sections I and II are completed by the offeror or subcontractor and provided to government assessors who had direct knowledge of the contractor's prior performance. These sections capture client identification and current or historical contract identification details including contractor name, contract number, contract type, period of performance, contract values, and a brief description of services provided. Sections III through VI are completed entirely by the government assessor. Section III requests the assessor's contact information and role. Section IV provides standardized performance level definitions ranging from Exceptional (E) to Unsatisfactory (U), with EXCEPTIONAL representing performance that meets contractual requirements and exceeds many to the government's benefit, while UNSATISFACTORY represents performance that does not meet most contractual requirements. Section V contains a Mission Capability evaluation table where assessors assign performance ratings using the defined scale and provide rationale for exceptional or unsatisfactory ratings. Section VI provides space for general comments. Completed questionnaires must be submitted to Laura Mabe at laura.s.mabe.civ@mail.mil by the proposal due date. The questionnaire is marked as containing Controlled Unclassified Information (CUI) and source selection sensitive information.
