About this file
This is a Business Proposal Attachment E form for RFP 27-88033 issued by the State of Indiana, Department of Administration (IDOA). The document serves as an instruction guide for respondents to submit comprehensive business proposal information across multiple sections, though specific details regarding products, services, quantities, response dates, bid deadlines, site visits, bidder conferences, award dates, and contract terms are referenced as being located in other sections of the RFP rather than detailed within this attachment itself.
Respondents must provide extensive documentation including company structure and organizational charts, financial stability evidence through Dunn & Bradstreet reports or audited financial statements for the two most recently completed fiscal years, and a certification of financial reporting accuracy signed by the CEO and/or CFO. The proposal requires three client references submitted directly to idoareferences@idoa.in.gov on the proposal submission due date, proof of registration to do business with the Indiana Secretary of State, evidence of corporate authorization, and documentation of efforts to engage with Indiana Veteran Owned Small Business (IVOSB) subcontractors or an explanation for declining such partnership. Additional requirements include submission of general company information such as FTIN, employee count, years of experience, and revenue data; descriptions of experience serving state governments and similar-sized clients; business continuity and disaster recovery plans; information security protocols; and acceptance or redlined comments regarding cloud-based Additional Terms and Conditions (IaaS, PaaS, or SaaS). Respondents must also indicate willingness to extend awarded pricing to other governmental bodies as outlined in the RFP.
