About this file
This is a Summary of Benefits and Coverage (SBC) document for the MSI Medica Choice Passport ASO 5000-20% HSA health plan, effective beginning January 1, 2021, prepared for Dunn County, Wisconsin in response to RFP #2021-03 Health and Wellness Services. The document outlines a PPO health plan covering individual and family enrollment with comprehensive medical benefits including preventive care, specialty services, emergency care, mental health services, maternity care, and rehabilitation services. The plan features an in-network deductible of $5,000 per person/$10,000 per family and an out-of-network deductible of $10,000 per person/$20,000 per family. Covered services are subject to coinsurance of 20% for in-network providers and 50% for out-of-network providers after the deductible is met. The plan covers preventive services and prenatal care before the deductible is satisfied. Notable limitations include a maximum of 15 combined chiropractic and acupuncture visits annually, and children's eye exams covered at no charge with eyeglass/contact lens coverage limited to $200 per two-year period.
The plan's out-of-pocket maximum is $7,000 per person/$14,000 per family for in-network services and $15,000 per person/$30,000 per family for out-of-network services. Excluded services include adult dental care, cosmetic surgery, bariatric surgery, infertility treatment, long-term care, and routine adult eye care. The plan is administered by Medica Self Insured and is classified as self-funded group health insurance meeting Minimum Essential Coverage and Minimum Value Standards. Network providers can be verified through www.medica.com or by calling 1-800-952-3455. Plan members do not require referrals to see specialists, and the plan accommodates HSA-eligible accounts at a 20% coinsurance rate for most services.

NOTE: Information about the cost of this plan (called the premium) will be provided separately. This is only a summary. For general definitions of common terms, such as allowed amount, balance billing, coinsurance, copayment, deductible, provider, or other underlined terms, see the Glossary. Important Questions Answers Why This Matters:
What is the overall deductible?
$5,000 per person/ $10,000 per family in-network and $10,000 per person/ $20,000 per family for out-of-network services.
Generally, you must pay all of the costs from providers up to the deductible amount before this plan begins to pay. If you have other family members on the plan, each family member must meet their own individual deductible until the total amount of deductible expenses paid by all family members meets the overall family deductible.
Are there servicescovered before you meetyour deductible?
Yes. Preventive care and prenatal care from in-network providers.
This plan covers some items and services even if you haven’t yet met the deductible amount. But a copayment or coinsurance may apply. For example, this plan covers certain preventive services without cost sharing and before you meet your deductible. See a list of covered preventive services at https://www.healthcare.gov/coverage/preventive-care-benefits.
Are there other deductibles for specific services? No. You don’t have to meet deductibles for specific services.
What is the out-of-pocket limit for this plan?
$7,000 per person/ $14,000 per family in-network. $15,000 per person/ $30,000 per family for out-of-network services.
The out-of-pocket limit is the most you could pay in a year for covered services. If you have other family members in this plan, they have to meet their own out-of-pocket limits until the overall family out-of-pocket limit has been met.
What is not included inthe out-of-pocket limit?
Premiums, balance billing charges, and health care this plan doesn’t cover.
Even though you pay these expenses, they don’t count toward the out-of-pocket limit.
Will you pay less if youuse a network provider?
Yes. See www.medica.com or call 1-800-952-3455 or 711 (TTY users) for a list of Medica Choice with UnitedHealthcare network providers.
This plan uses a provider network. You will pay less if you use a provider in the plan’s network. You will pay the most if you use an out-of-network provider, and you might receive a bill from a provider for the difference between the provider’s charge and what your plan pays (balance billing). Be aware, your network provider might use an out-of-network provider for some services (such as lab work). Check with your provider before you get services.
Do you need a referral tosee a specialist? No. You don’t need a referral to see a specialist. You can see the specialist you choose without a referral.
