About this file
This is a Summary of Benefits and Coverage (SBC) document for the MSI Medica Choice Passport ASO 3000-20% HSA health plan, a preferred provider organization (PPO) plan administered by Medica Self Insured for coverage beginning January 1, 2021. The plan covers individual and family enrollment with specific cost-sharing structures for in-network and out-of-network services. In-network services require a $3,000 individual deductible or $6,000 family deductible, while out-of-network services carry a $6,000 individual or $12,000 family deductible. The plan establishes out-of-pocket limits of $6,000 individual/$12,000 family for in-network services and $12,000 individual/$24,000 family for out-of-network services. Covered services include primary care, specialist visits, preventive care, diagnostic testing, prescription drugs, emergency care, hospitalization, mental health services, maternity care, and rehabilitation services, with most services subject to 20% coinsurance for in-network providers and 40% for out-of-network providers after the deductible is met.
The plan excludes coverage for adult dental care, adult routine eye care, bariatric surgery, cosmetic surgery, infertility treatment, long-term care, and weight loss programs. Chiropractic and acupuncture services are limited to 15 combined visits annually. Prescription drug coverage includes generic drugs, preferred and non-preferred brand drugs, and specialty drugs at specified coinsurance levels, with mail order supplies available up to 93 days. Children's eye exams are covered at no charge with a $200 maximum benefit every two years for eyeglasses and contact lenses. The plan does not require referrals for specialist visits and covers preventive services without cost-sharing before deductible requirements are met. Additional contact information is provided for Medica at 952-945-8000 or 1-800-952-3455, with grievance and appeals processes available through the plan administrator and state regulatory agencies.

MSI Medica Choice Passport ASO 3000-20% HSA Coverage Period: Beginning on or after 1/1/21 Summary of Benefits and Coverage: What this Plan Covers & What You Pay for Covered Services Coverage for: Individual/Family | Plan Type: PPO
The Summary of Benefits and Coverage (SBC) document will help you choose a health plan. The SBC shows you how you and the plan would share thecost for covered health care services. NOTE: Information about the cost of this plan (called the premium) will be provided separately. This is only a summary. For more information about your coverage, or to get a copy of the complete terms of coverage, go to www.medica.com or call 952-945-8000 (Minneapolis/St. Paul Metro area) or 1-800-952-3455. For general definitions of common terms, such as allowed amount, balance billing, coinsurance, copayment, deductible, provider, or other underlined terms, see the Glossary. You can view the Glossary at www.healthcare.gov/sbc-glossary or call Medica at 1-800-952-3455 to request a copy.
Important Questions Answers Why This Matters:
What is the overall deductible?
$3,000 per person/ $6,000 per family in-network and $6,000 per person/ $12,000 per family for out-of-network services.
Generally, you must pay all of the costs from providers up to the deductible amount before this plan begins to pay. If you have other family members on the plan, each family member must meet their own individual deductible until the total amount of deductible expenses paid by all family members meets the overall family deductible.
Are there servicescovered before you meetyour deductible?
Yes. Preventive care and prenatal care from in-network providers.
This plan covers some items and services even if you haven’t yet met the deductible amount. But a copayment or coinsurance may apply. For example, this plan covers certain preventive services without cost sharing and before you meet your deductible. See a list of covered preventive services at https://www.healthcare.gov/coverage/preventive-care-benefits.
Are there other deductibles for specific services? No. You don’t have to meet deductibles for specific services.
What is the out-of-pocket limit for this plan?
$6,000 per person/ $12,000 per family in-network. $12,000 per person/ $24,000 per family for out-of-network services.
The out-of-pocket limit is the most you could pay in a year for covered services. If you have other family members in this plan, they have to meet their own out-of-pocket limits until the overall family out-of-pocket limit has been met.
What is not included inthe out-of-pocket limit?
Premiums, balance billing charges, and health care this plan doesn’t cover.
Even though you pay these expenses, they don’t count toward the out-of-pocket limit.
Will you pay less if youuse a network provider?
Yes. See www.medica.com or call 1-800-952-3455 or 711 (TTY users) for a list of Medica Choice with UnitedHealthcare network providers.
This plan uses a provider network. You will pay less if you use a provider in the plan’s network. You will pay the most if you use an out-of-network provider, and you might receive a bill from a provider for the difference between the provider’s charge and what your plan pays (balance billing). Be aware, your network provider might use an out-of-network provider for some services (such as lab work). Check with your provider before you get services.
Do you need a referral tosee a specialist? No. You don’t need a referral to see a specialist. You can see the specialist you choose without a referral.
