About this file
This is a company information template for the STE VSW (Sources Taught, Evaluated, Validated, and Supported) program, designed to collect organizational and registration details from prospective contractors.
The form requests comprehensive company identification data including legal company name, business address, and a designated point of contact with email and phone number for contract matters. It also requires registration identifiers such as CAGE code and UEI (Unique Entity Identifier), along with the expiration date of SAM (System for Award Management) registration status. The template captures compliance and eligibility information, specifically requesting confirmation of Wide Area Workflow registration status, Non-Traditional Defense Contractor (NDC) designation under 10 USC 4022, small business classification, and debarment or suspension status. The document includes a detailed footnote defining NDC status as an entity that has not performed any Department of Defense contract or subcontract subject to full cost accounting standards coverage within the one-year period preceding the solicitation.
