About this file

This is a Health Insurance Rates Information document for the City of Holly Hill, Florida, detailing employee and retiree benefit premiums across medical, dental, and vision coverage for the 2024-2026 contract period. The City of Holly Hill provides comprehensive health insurance coverage to approximately 109 active employees and 53 retirees through Florida Health Care Plans (HDHP medical plan with HSA eligibility), Principal (dental), and EyeMed (vision). The rates document establishes semi-monthly premium costs for four coverage tiers—employee only, employee/spouse, employee/children, and employee/family—with corresponding City and employee contribution portions. Notably, employees are eligible to earn up to $1,900 annually in HSA contributions through completion of the City's Annual Wellness Initiative requirements. A non-mandatory pre-proposal conference for broker services is scheduled for March 18, 2026, at 3:00 PM; written questions must be submitted by 5:00 PM EST on April 2, 2026; sealed proposals are due by 3:00 PM EST on April 8, 2026; mandatory City Commission presentations are scheduled for April 28, 2026; and award is anticipated for May 12, 2026, with coverage effective July 1, 2026. The initial contract term is two years with renewal options for up to three additional one-year terms.

Medical premiums for the HDHP plan feature a $1,700 individual and $3,400 family deductible with corresponding out-of-pocket maximums of $3,400 and $6,800 respectively. As of January 1, 2026, monthly HDHP premiums range from $641.10 for employee-only to $1,346.31 for family coverage, with the City bearing $641.10 per employee regardless of coverage tier. Dental coverage includes 100% preventive services, 80% basic procedures, and 50% major procedures capped at $5,000 annually, with 2026 premiums ranging from $35.41 to $127.75 monthly. Vision premiums remain stable at $4.86 to $14.26 monthly across all contract years, providing $10 copays for exams, $25 copays for lenses, and $100 frame allowances for in-network services. The City currently provides basic group term life insurance at no employee cost through USAble Life with voluntary coverage available up to $500,000 or five times salary. The City's incumbent carriers—Florida Health Care Plans, Principal, EyeMed, and USAble Life—must maintain A.M. Best ratings of A or higher, and the selected broker must maintain professional liability insurance of $5,000,000 minimum and commercial general liability of $500,000 per occurrence.
