About this file

This document is a daily health screening inventory for COVID-19. It contains questions for employees to self-assess any COVID-19 symptoms they may be experiencing. Employees are asked if they currently have or have experienced in the past 10 days symptoms such as fever, chills, fatigue, cough, shortness of breath, or loss of taste/smell. It also inquires if the employee is awaiting COVID-19 test results or has been told to isolate or quarantine. Additional questions determine if an employee lives with someone experiencing COVID-19 symptoms or has been in direct contact with a confirmed case. The inventory instructs employees to notify their work center monitor of their responses before arriving to work and to stay home if answering yes to any questions.

The related federal contract opportunity is a special notice for the AOC Multi-Award E/A IDIQ with solicitation number AOCACB23R0122 issued by the Architect of the Capitol agency. As a special notice, it does not provide pricing terms, response dates, products/services required, or other salient details about the procurement.
