About this file
This is Amendment 3 to RFP 2027-1200-0020 issued by the State of Alaska Department of Public Safety, Division of Administrative Services, addressing clarifications regarding applicant medical examination services. The RFP seeks contractor services for pre-employment medical examinations, fitness-for-duty evaluations, and drug testing for Alaska State Trooper Recruits, Court Services Officers, and Deputy Fire Marshals. The scope includes medical history reviews, physical assessments with vision and hearing tests, cardiovascular and musculoskeletal examinations, urinalysis, tuberculosis screening, six-panel drug profile testing, and breath alcohol testing. The contract requires 120 pre-employment screening and evaluation instances and 5 fitness-for-duty evaluations per contract term, with services provided in Anchorage and Fairbanks. The proposal deadline was revised to 1:30 p.m. Alaska Standard Time on August 19, 2026. The initial contract term begins approximately September 1, 2026, with four optional one-year renewal periods at the state's sole discretion.
Amendment 3 clarifies that licensed physicians, advanced practice registered nurses, and physician assistants are all permitted to certify and sign the required APSC F2A-B medical examination forms. The estimated 120 pre-employment hours and 5 fitness-for-duty hours represent annual utilization projections based on average testing cycles, with hourly rates expected to encompass all required examination components including drug testing. The state prefers a permanent physical clinic location in Anchorage at minimum, though contractor-employed personnel may provide services at compliant clinical locations. Appointment availability should be provided within approximately four weeks of referral, with examination reports and fitness-for-duty reports due within two weeks at latest. The contract is firm fixed-price with pricing subject to available appropriated funds, and all work must be performed by the contractor's own staff with no subcontractors or joint ventures permitted. Amendments 1, 2, and 3 are mandatory returns requiring authorized signatures with the proposal submission.

Questions submitted by potential offerors and answers from the state:
Question 1: Section 3.01(e) states that APSC Form F2-B may be certified by a physician, physician assistant, or advanced practice registered nurse; however, page 11 states that the F2A-B must be certified by a licensed physician. Please clarify which provider types are permitted to perform the pre-employment medical examination and which provider type(s) may certify/sign the APSC F2A-B.
Answer: The regulation states “certified by a licensed physician, advanced practice registered nurse, or physician assistant.”
Question 2: The Cost Proposal identifies 120 estimated hours for Pre-employment Screening / Examination / Evaluation Services and 5 estimated hours for Fitness-for-Duty Evaluation Services per contract term. Are these estimated hours intended solely for proposal evaluation purposes, or do they represent anticipated annual utilization?
Answer: 120 estimated hours for Pre-employment Screening / Examination / Evaluation Services and 5 estimated hours for Fitness-for-Duty Evaluation Services per contract term is an estimated number of evaluations based on average testing cycles.
Question 3: For the Pre-employment Screening/Examination/Evaluation line on the Cost Proposal, please clarify whether the hourly rate is expected to include all required components of the examination, including the 6-panel drug profile, non-DOT breath alcohol test, urinalysis, and TB screening.
Answer: Yes, 120 would be the estimated people per year, which would include the exam and drug test screening.
Question 4: Does DPS have historical information regarding the approximate number of pre-employment examinations and fitness-for-duty evaluations performed annually?
Answer: Yes.
Question 5: Does the contractor need to maintain a permanent physical clinic location in both Anchorage and Fairbanks, or may services be provided by contractor-employed personnel at another compliant clinical location?
Answer: It would be preferred that there is a permanent physical clinic in Anchorage, at least.
Question 6: Is there a required timeframe from referral to appointment availability for pre-employment examinations and fitness-for-duty evaluations?
Answer: We strive to refer applicants to the vendor 4 weeks prior to testing; however, in some cases there are edits that we may make up to a week prior to testing week. Fitness for duties would be on an as needed basis as soon as vendor is available upon request.
Question 7: Is there a required turnaround time for submission of completed examination reports and fitness-for-duty reports?
Answer: As soon as possible, but within 2 weeks at latest is preferred.
