This is Amendment 1 to RFP 2027-1200-0020 issued by the State of Alaska Department of Public Safety, Division of Administrative Services for Applicant Medical Examinations. The amendment extends the bid deadline from August 17, 2026, at 1:30 p.m. AKST to August 19, 2026, at 4:30 p.m. AKST. The RFP solicits proposals for pre-employment medical examinations, fitness-for-duty evaluations, and drug testing services to be provided in two locations: Anchorage and Fairbanks. Services include medical history reviews, physical assessments with vision and hearing tests, cardiovascular evaluations, musculoskeletal examinations, laboratory urinalysis, tuberculosis screening, drug profile testing, and breath alcohol testing for Alaska State Trooper Recruits, Court Services Officers, and Deputy Fire Marshals. The contract requires 120 pre-employment screening and evaluation instances and 5 fitness-for-duty evaluations per contract period. The initial contract term begins approximately September 1, 2026, with four optional one-year renewal periods at the state's sole discretion. Notice of intent to award is anticipated for the week of August 24, 2026. All questions must be submitted in writing to Procurement Specialist Calin Logan at (907) 269-0599 or calin.logan@alaska.gov, and offerors must sign and return the amendment page with their proposals.

Qualified individual practitioners must be practicing adult primary care physicians, physician assistants, or advanced practice registered nurses with a minimum of two years' recent experience performing medical evaluations and fitness-for-duty assessments within the previous four years; proposals failing to meet this requirement will be rejected as non-responsive. Contractors must hold valid Alaska business licenses and applicable professional licenses prior to the proposal deadline and maintain commercial general liability insurance at $300,000 minimum, commercial automobile liability at $300,000 minimum, and professional liability based on contract value. Cost proposals must include all direct and indirect costs such as personnel, licensing, hardware, supplies, overhead, and profit. Payment is due within 30 days of invoice receipt, with a 5% discount available for invoices paid within 15 business days. Evaluation criteria allocate 40% to qualifications and understanding, 50% to contract cost, and 10% to Alaska Offeror Preference. No subcontractors or joint ventures are permitted.
