Except as provided herein, all terms and conditions of the document referenced in Item 9A or 10A, as heretofore changed, remains unchanged and in full force and effect.

STATEMENT OF WORK

GENERAL

A. Contractor shall be responsible for all work associated with relocating the existing CXT to the newly constructed parking lot site.

B. Contractor shall coordinate pumping of the existing CXT tank prior to moving.

C. Contractor shall contact CXT for their requirements and recommendations for moving and resetting the existing CXT.

D. After CXT installation, backfill and compact according to the plans.

E. The Marion Forks fish hatchery and campground shall remain open to the public during construction activities. Work zone shall be clearly marked, safety barriers shall be put in place to warn the public, and a section of the road shall be closed to the public.

PROJECT SUMMARY

A. Prior to initiate any construction work, the contractor shall submit a Written Site Specific Safety Plan. The Safety Plan shall include measures to protect workers and the public during construction activities. This plan shall be approved by the CO prior to initiate any construction work.

B. Prior to commencement of construction operations, contractor shall implement the Site Specific Safety Plan.

C. Prior to construction, contractor shall locate all underground utilities (e.g. water, sewer, and electricity) and improvements within the project work limits and protect or properly abandon them as needed.

D. Contractor shall clear, grub, grade, and prepare the work site for installation of the CXT Vault Toilet.

E. Contractor shall construct a new walkway around the new CXT building unit.

Walkway shall conform to the dimensions and characteristics stated in the drawings and specifications.

F. Where manufacturer’s recommendations, specific field conditions, industry standards, State regulations, building codes, etc. conflict with the requirements of the Contract Specifications or Drawings, immediately notify and work with the CO to resolve the conflicting specifications. The CO will have the final decision on which requirements shall apply.

G. Contractor shall produce a Work Progress Schedule to show when field work is expected to occur. Work shall be completed during the contract period, and before the end of September 30, 2014.

Association, a state agency, or other acceptable organization. Perform the work described under MUTCD Part 6. Use type III, VII, VIII, or IX retro reflective sheeting on flagger paddles. Do not use flags. Flaggers must wear high visibility safety apparel as required by MUTCD 6E.02.

156.05 Temporary Closures. Road segments may be closed during work hours as shown in Table 156-1; however, road 2200-050 must be open to the public at the end of each day. During the daily closure, the Contractor shall only be required to provide access for emergency situations and fish hatchery personnel. Contractor is encouraged to discuss with Greg Grenbemer, hatchery manager, at (503) 854-3522 for their required access and needs. The maximum consecutive days of closure shall be followed by a minimum number of consecutive days open to traffic as shown. Maintain traffic control devices during closure period(s).

Appropriate barricades and signs will be erected and maintained as shown in the traffic control plan or as otherwise designated.

Prior to closing roads during construction, give written notice to the Contracting Officer at least 7 days in advance.

157.03 General

Delete the entire subsection and replace with the following:

Prior to the start of construction, submit a written plan that provides permanent and temporary erosion control measures to minimize erosion and sedimentation during and after construction.

Do not begin work until the necessary controls for that particular phase of work have been implemented. Do not modify the type, size, or location of any control. An alternate erosion control plan with all necessary permits may be submitted 30 days before intended use.

Questions and Answers Marion Forks 6/4/14

1. In the specs, it calls out the allowable time for delays to traffic and also states that the facilities must remain accessible throughout the construction project. Is it the Governments position to keep the facilities (i.e. road and fish hatchery) open seven days a week at all times. (other than the allowable shut down periods) For example, will we be able to close the entrance road on the day of paving?

Answer: Please refer to the solicitation document page 19. Also see Supplemental specifications on page 16, 156.05 Temporary Closures.

2. According to the plans there will be a power transformer that needs to be relocated. Will the Government be responsible for the associated fees, if any, charge by the utility provider?

Answer: No, the contractor is required to pay fees to CPI (utility provider). See general notes item 15101 (Drawings page 3, as amended).

3. Who is responsible for physically moving the CXT structure?

Answer: The contractor will be responsible for all work necessary to move the existing CXT structure (see amended CSI specifications page 2).

4. Could you send a copy of the plans that are on FBO?

Answer: To maintain fairness, we do not supply printed documents to any of the interested vendors—all have equal access.

5. What labor percentage must be done by the hub-zone prime contractor?

Answer: FAR clause 52.219-3 establishes the following percentages:

(3) General construction.

(i) At least 15 percent of the cost of contract performance to be incurred for personnel will be spent on the HUBZone prime contractor’s employees;

(ii) At least 50 percent of the cost of the contract performance to be incurred for personnel will be spent on the HUBZone prime contractor’s employees or on a combination of the HUBZone prime contractor’s employees and employees of HUBZone small business concern subcontractors; and

(iii) No more than 50 percent of the cost of contract performance to be incurred for personnel will be subcontracted to concerns that are not HUBZone small business concerns.

If your company does not have the capacity to meet these percentages, please contact me via email before the solicitation closes.

6. Will the contractor be responsible for any tree removal on the job?

Answer: Yes, see general notes item 20105 and appropriate supplements for parking lot work. For tree removal for trail, refer to clearing and grubbing sheet 18 of plans and section 911 of the trail supplemental specifications.

7. Will the contractor be responsible for the removal of the existing pile of boulders in the trail area?

Answer: No, we have a stream restoration project which will include moving and using those boulders prior to this project beginning.

8. There is no bid item for construction survey and staking in the schedule of items for trail construction (B.1C), who is responsible for surveying the trail construction work?

Answer: The contractor will be responsible. Refer to the R6 trail specs, section 908.

However, since those specifications do not mention surveying, this will be paid under 15201 (see Drawings page 4 as amended).

9. At the Minto Gravel Pit there are some large piles of screened rock, can this be used on the job or do we have to excavate and screen from the hillside?

Answer: No, existing stockpiles of aggregate are not to be used for the project. Any material obtained from the Minto pit will require the contractor to generate and sort it.

Suitable material excavated from the roadway work may be used (see Drawings page 3, as amended)
