STATEMENT OF WORK

GENERAL

A. The Detroit Ranger District, Willamette National Forest, will move an existing CXT toilet at the Marion Forks Day Use area to a newly constructed parking lot site located within 500 ft of existing site.

B. Contractor shall coordinate pumping of the existing CXT tank prior to moving.

C. Contractor shall contact CXT for their requirements and recommendations for moving and resetting the existing CXT.

D. After CXT installation, backfill and compact according to the plans.

E. The Marion Forks fish hatchery and campground will remain open to the public during construction activities. Work zone shall be clearly marked, safety barriers shall be put in place to warn the public, and a section of the road shall be closed to the public.

PROJECT SUMMARY

A. Prior to initiate any construction work, the contractor shall submit a Written Site Specific Safety Plan. The Safety Plan shall include measures to protect workers and the public during construction activities. This plan shall be approved by the CO prior to initiate any construction work.

B. Prior to commencement of construction operations, contractor shall implement the Site Specific Safety Plan.

C. Prior to construction, contractor shall locate all underground utilities (e.g. water, sewer, and electricity) and improvements within the project work limits and protect or properly abandon them as needed.

D. Contractor shall clear, grub, grade, and prepare the work site for installation of the CXT Vault Toilet.

E. Contractor shall construct a new walkway around the new CXT building unit.

Walkway shall conform to the dimensions and characteristics stated in the drawings and specifications.

F. Where manufacturer’s recommendations, specific field conditions, industry standards, State regulations, building codes, etc. conflict with the requirements of the Contract Specifications or Drawings, immediately notify and work with the CO to resolve the conflicting specifications. The CO will have the final decision on which requirements shall apply.

G. Contractor shall produce a Work Progress Schedule to show when field work is expected to occur. Work shall be completed during the contract period, and before the end of September 30, 2014.

SECTION 00100

MEASUREMENT AND PAYMENT

PART 1 – GENERAL

1.1 MEASUREMENT AND PAYMENT

A. Measurement and payment for contract work will be made only for and under those pay items included in the SCHEDULE OF ITEMS. All other work and materials will be considered as included in the payment for items shown.

B. When more than one class, size, or thickness is specified in the SCHEDULE OF ITEMS for any pay item, suffixes will be added to the item number to differentiate between items to be bid.

1.2 DETERMINATION OF QUANTITIES

A. For individual construction items, longitudinal and lateral measurements for area computations will be made horizontally or corrected to horizontal measurement unless otherwise specified.

B. The average end area method will be used to compute volumes of excavation or embankment. However, if in the judgment of the COR, the average end area method is impractical, measurement will be made by volume in hauling vehicles or by other three dimensional methods.

C. Structures will be measured according to lines SHOWN ON THE DRAWINGS or as altered by the CO in writing to fit field conditions.

D. For items that are measured by the linear foot, such as pathway, measurements will be made parallel to the base or foundation upon which the structures are placed.

1.3 UNITS OF MEASUREMENT

A. Payment will be by units defined and determined according to U.S.

Standard measure as follows:

1. Acre – Make longitudinal and transverse measurements for area computations horizontally.

2. Cubic Yard - A measurement computed by one of the following methods:

a. Excavation, Embankment, or Borrow - The measurement computed by the average end area method from measurements made longitudinally along a centerline or reference line.

b. Material in Place or Stockpile - The measurement computed with dimensions of the in-place material.

c. Material in the Delivery Vehicle - The measurement computed using measurements of material in the hauling vehicles at the point of delivery. Vehicles shall be loaded to at least their water level capacity. Leveling of the loads may be required when vehicles arrive at the delivery point.

3. Cubic Yard Mile - A combination of linear and volumetric measurement meaning the movement of a cubic yard of material 1 mile.

4. Each - One complete unit, which may consist of one or more parts.

The quantity is the actual number of units completed and accepted.

5. Gallon – The quantity may be measured by any of the following methods:

a. Measured volume container.

b. Metered volume. Use an approved metering system

c. Commercially packaged volumes.

6. Hour – Measurement will be for the actual number of hours ordered and performed by the Contractor.

7. Linear Foot – Measure from end to end, parallel to the base or foundation being measured, or horizontal.

8. Lump Sum – Do not measure directly. The bid amount is complete payment for all work described in the contract and necessary to complete the work for that item.

9. MFBM - One thousand board feet measure based on nominal widths, thicknesses, and extreme usable length of each piece of lumber or timber actually incorporated in the job.

10. Mile – Measure horizontal along the centerline of each roadway, approach road, or ramp.

11. Pound – If sacked or packaged material is furnished, the net weight as packed by the manufacturer may be used.

12. Square Foot - Measure on a plane parallel to the surface being measured or horizontal.

13. Square Yard – Measure on a plane parallel to the surface being measured or horizontal.

14. Station - One hundred linear feet measured horizontally.

15. Station Yard - A combination of linear and volumetric measurement meaning the movement of a cubic yard of material one station.

16. Thousand Gallons Mile - A combination of linear and volumetric measurement meaning the movement of 1,000 gallons of material 1 mile.

17. Ton - Short ton consisting of 2,000 pounds.

18. Ton Mile - A combination of linear and weight measurement meaning the movement of 1 ton of material 1 mile.

1.4 METHODS OF MEASUREMENT

A. One of the following methods of measurement for determining final payment is DESIGNATED on the SCHEDULE OF ITEMS for each pay item:

1. Designed Quantities (DQ) - These quantities denote the final number of units to be paid for under the terms of the contract. They are based upon

Original design data include the preliminary survey information, design assumptions, calculations, drawings, and the presentation in the contract.

Changes in the number of units SHOWN in the SCHEDULE OF ITEMS may be authorized under any of the following conditions:

a. As a result of changes in the work authorized by the CO.

b. As a result of the CO determining that errors exist in the original design data used to determine designed quantities that cause a pay item to change by 15 percent or more.

c. As a result of the Contractor submitting to the CO a written request showing evidence of errors in the original design data used to determine design quantities that cause a pay item total to change by 15 percent or more. The evidence must be verifiable and consist of calculations, drawings, or other data that show how the designed quantity is believed to be in error.

2. Staked Quantities (SQ) - These quantities are determined from staked measurements prior to construction.

3. Actual Quantities (AQ) - These quantities are determined from measurements of completed work.

4. Vehicle Quantities (VQ) - These quantities are measured or weighed in hauling vehicles.

5. Lump Sum Quantities (LSQ) - These quantities denote one complete unit of work as required by or described in the contract, including necessary materials, equipment, and labor to complete the job. They will not be measured.

SECTION 01300

SUBMITTALS

1.1 SUMMARY

A. The following submittal requirements and procedures are required in addition to and as an explanation of FAR Clause 52.236-21 and AGAR Clause 452.236-76.

B. Section Includes:

1. These requirements are for all submittals including but not limited to, product data, samples, shop drawings, catalogs and manufacturer’s literature, schedules, substitution requests, layouts, as-built drawings, maintenance and operating data, guarantees, etc.

2. Submittals shall be made for all fabricated items furnished and installed as specified.

3. Work shall not commence on any item requiring submittals until submittals are approved in writing by the CO.

1.2 SUBMITTAL REQUIREMENTS

A. All submittals shall be accompanied by a submittal transmittal form.

B. Equipment numbers shall be listed for items being submitted.

C. Submittals for various items shall be made with a single form when the items taken together constitute a manufacturer’s package or are so functionally related that expediency indicates checking or review of the group or package as a whole.

D. Submit three sets of catalog cuts, shop drawings and manufacturer's installation, operating and maintenance instructions for all material and equipment requiring submittals to the CO.

E. Obtain written approval for the above submittals prior to construction, assembly, and delivery.

F. Submittals shall have sufficient detail to determine if ratings, characteristics, and physical construction are equivalent to those specified.

G. Where manufacturer's catalog sheets are used, indicate the specific items to be supplied in ink either by arrow, encircled on the sheet, or the inapplicable portion blocked out. List manufacturer's name, contract, section and drawing sheet reference number on each catalog sheet. Identify where equipment is to be used, if not obvious.

H. Submittals shall show any changes to be made resulting from substitution of equipment or materials for those that are specified.

1.3 TYPE AND NUMBER REQUIRED:

A. Manufacturer’s Literature Submit three (3) each copies of required manufacturer’s literature. Modify product data by deleting information which is not applicable to the project, or by marking each copy to identify

B. Correcting: The Contractor shall make any corrections required by the CO and shall resubmit corrected submittals until accepted. The Contractor shall provide for enough time for reviews, revisions and re-submittals to not delay the work.

1.4 SUBMITTALS

A. Site Specific Safety Plan: To ensure a safe work environment for workers and the public.

B. Work Progress Schedule: To ensure adequate planning and execution of the

Work so that the Work is completed within the respective number of calendar days allowed in the contract, and to assist the CO in appraising the reasonableness of the proposed schedule and evaluating the progress of the Work, prepare the following:

1. Within ten (10) calendar days after being awarded the Contract, prepare and submit for the CO’s information, a progress schedule for the Work using estimated times and events, if necessary. The progress schedule shall be related to the entire project to the extent required by the Contract Documents, and shall provide for expeditious and practicable execution of the work.

2. Update the schedule as needed and resubmit it to the CO for reference with each progress payment request.

1.5 CONTRACTOR’S VERIFICATION

A. General

1. All submittals are instruments of the Contractor. The Contractor shall carefully check all items of the Contractor’s subcontractors and supplier as well as the Contractor’s own before transmitting them as the Contractor’s submittals to the CO.

B. Determine and verify:

1. Field measurements.

2. Field construction criteria.

3. Catalog numbers and similar data.

4. Conformance with specifications.

C. Stamp and initial all submittals indicating that they have been reviewed prior to sending to CO.

D. QUESTIONS: The Contractor shall review all submittals and include answers to any questions or clarifications required by subcontractors and, in particular, shall ascertain that the submittals meet all requirements of the Contract Documents.

1.6 COORDINATION OF SUBMITTALS

A. GENERAL: Prior to submittal for CO’s review, use all means necessary to fully coordinate all material, including the following procedures:

1. Determine and verify all field dimensions and conditions, materials, catalog numbers and similar date.

2. Coordinate as required with all trades and with all public agencies involved.

3. Secure all necessary approvals from public agencies and others and signify by stamp, or other means, that they have been secured.

4. Clearly indicate all deviations from the Contract Documents.

5. On re-submittals, clearly indicate all revision or changes from original submittal.

B. GROUPING OF SUBMITTALS: Unless otherwise specifically permitted by the CO make all submittals in groups containing all associated items; the CO may reject partial submittals as not complying with the provisions of the Contract Documents.

1.7 TIME SCHEDULES

A. GENERAL

1. Make all submittals far enough in advance of schedule dates of installation to provide all required time for reviews, for securing necessary approvals, for possible revisions and re-submittal, and for placing orders and securing delivery.

2. In scheduling, allow at least (10) calendar days for the CO’s review following receipt of each submittal. On re-submittals, allow at least 7 calendar days.

3. Though the CO may utilize other agents (inside or outside the USFS) to assist in submittal review, only the CO has the authority to issue action in regards to submittals.

B. DELAYS: The Contractor shall provide for enough time for reviews, revisions and re-submittals to not delay the work. Costs of delays occasioned by tardiness of submittals shall be borne by the Contractor.

C. SHOP DRAWINGS AND PRODUCT DATA

1. The Contractor shall make any corrections required by the Government and shall resubmit corrected Shop Drawings, and manufacturer’s literature until accepted.

A. General.

1. Grade around building walkways to the appropriate contours and elevations of the site and smoothly blend into existing contours. Grade to uniform levels or slopes between points where grades or contours are given, round surfaces at abrupt changes in levels. Slope ground away from building walls, walkways, and pathway to facilitate drainage.

2. Hand rake areas within the clearing limit lines. Remove all roots, brush, weeds and stones larger than 2 inches from the finish graded surface.

B. Grading Around Building.

1. Grade all areas to produce uniform slopes between spot elevations and contours.

2. Control grading within 5 feet of building and all other structures to prevent water from running into excavated areas. Remove all water

C. Excess Material.

CRUSHED AGGREGATE

1.1 DESCRIPTION

A. This work consists of utilizing excavated roadway sub-base material for creating access pathway around the CXT vault toilet.

PART 2 – PRODUCTS

2.1 CONFORM TO THE FOLLOWING SPECIFICATIONS

1. Obtain aggregate from material under existing asphalt road 2200-050.

PART 3 – EXECUTION

3.1 PREPARING SURFACE

A. Shape, compact and finish the bedding surface to the required lines, grades, and cross-sections by the standards describe in Drawings and Specifications.

B. Mix the aggregate and adjust the moisture content to obtain uniform moisture content suitable for compaction. Spread and shape the mixture on the prepared surface in a uniform layer.

3.2 PLACING CRUSHED AGGREGATE

A. Place the mixture in a layer to yield a minimum of 4 inches in compacted thickness.

B. Compact with a minimum of five passes with a mechanical vibrating tamper over the full width of the entire layer.

3.3 ACCEPTANCE

A. Placement aggregate will be evaluated based on visual inspection of the work in compliance with the Contract and prevailing industry

SECTION 13129

SITE PREPARATION AND FINISHING WORK FOR PRECAST

CXT TOILET BUILDING

A. The work of this Section consists of relocating existing CXT. Work includes backfilling and compacting existing CXT location, moving existing CXT to new location, excavating and backfilling new CXT location site, and site cleanup.

1.2 RELATED SECTIONS

1.3 QUALITY ASSURANCE

A. Prior to moving the existing CXT vault toilet unit, contractor shall ensure that the site has been prepared according to drawings and specifications and manufactures recommendations.

B. The contractor shall ensure that walkway around the CXT unit are constructed according to drawings and specifications.

C. Contractor shall ensure that transitional area from the access pathway to the CXT unit building meets ADA requirements.

1.4 CXT INSTALLATION

A. Prior to installation of CXT unit. Contractor shall prepare the site conditions for the CXT unit according to DRAWINGS AND SPECIFICATIONS. Site preparation includes backfilling and compaction of the area occupied at the previous location, backfilling and compacting around area occupied at new location, and grading.

1.5 FINISHING WORK

A. Contractor shall finish work areas around the CXT unit according to drawings and specifications.

PART 2 – PRODUCTS

2.1 SOIL CLASSIFICATION

A. Excavation shall be unclassified as to materials and shall include all materials that are encountered in the required excavation.

2.2 BACKFILL MATERIAL

A. Backfill material shall be sandy clay, sand, gravel, soft shale, or other suitable material free from brush, organic material, dirt clods, stone or

B. Backfill material shall be excavated material whenever it meets specification requirements. Whenever excavated material contains less than 10 percent of oversized material, the Contractor shall remove boulders larger than 6” from the excavated material at no additional compensation and utilize it as backfill material. Whenever material meeting the specification requirements is not available from excavation, the Contractor shall import material from a designated or approved source.

C. Fill Material.

1. Material free from frozen or organic material, and rocks larger than 2 inches.

2.3 AGGREGATE BASE

A. Refer to Section 02748 – Crushed Aggregate

2.4 ACCESSIBILITY

A. Contractor shall ensure that the transitional area from the pathway accessing the new toilet facility is in compliance with ADA requirements.

PART 3 – EXECUTION

3.1 TOPSOIL

A. If top soil is encountered, it shall be removed from the area to be excavated and from the area where the material shall be piled prior to excavating.

Topsoil shall be kept separate from excavated material. Topsoil shall be spread on those areas that had been disturbed.

3.2 SAFETY, SHORING, AND PROTECTION

A. The Contractor shall meet OSHA safety rules and regulations.

B. Walls of excavations 4’ or more in depth shall be supported by bracing, shoring, or other methods, unless the walls are sloped to a safe enough angle from the bottom. If shored, the excavation shall be of proper dimensions to accommodate shoring and bracing, as required, to keep walls from collapsing and to allow for proper installation of the work.

C. All existing improvements shall be fully protected from damage.

D. All supports shall be removed after construction is completed, and shall be withdrawn in a manner that will prevent the collapse of the sides of the excavation, All openings in the ground, caused by the removal of supports, shall be filled with suitable material properly compacted.

E. All excavations left open overnight shall be fenced with wire or plastic mesh secured to steel posts all around the excavation.

1. The bottom of the fence shall follow the contour of the ground.

2. Maximum spacing of the steel posts shall be 10 feet.

3. Minimum height of the fence shall be 36 inches.

3.3 EXCAVATION AND BACKFILL

A. Excavation.

1. Excavation shall not begin until all clearing and grubbing work is

Marion Forks Day Use Area Willamette National Forest Detroit Ranger District Request for Proposals completed except for final debris disposal. Operations shall be in compliance with the requirements and restrictions of OSHA, and the Oregon Occupational Safety and Health Code.

B. Wet Excavation.

1. If water is encountered during trench excavation, provide adequate pumping equipment and piping to keep trench free from water and dispose of the water without damage to adjacent property. Separate payment will not be made for pumping of water from trenches or for excavation of material rendered unsuitable as a result of water encountered.

C. Unsuitable Material.

1. Where unsuitable material is encountered in the trench bottom, as determined by the CO, excavate the trench below grade to the depth specified by the CO.

D. Excavated Material.

1. Pile excavated material at the side of the trench where it will give the best protection for the work. Leave adequate space between the toe of the spoil pile and edge of trench to prevent material from falling into the trench.

E. Backfill.

1. Before compacting backfill other than rock or sand, prepare material to a water content that will permit proper compaction by either aerating the material if it is too wet, or spraying the material with water if it is too dry.

Thoroughly mix each lift before compaction to assure uniform distribution of water content. Do not compact material above pipe or cable until there are 6 inches of cover over the pipe or cable.

2. Compact each lift with the required number of passes of a mechanical vibrating tamper over the entire layer.

a. Select Material.

i. Place in 6 inches lifts or less and compact with a minimum of 3 passes over the entire lift surface.

ii.

3.4 CONSTRUCTION

A. General

1. Grade around building sidewalks and smoothly blend into existing contours. Slope ground away from building sidewalks at a 2% slope to facilitate drainage.

2. Hand rake areas within the grading limit lines. Remove all roots, brush, weeds and stones larger than 2 inches from the finish graded surface.

B. Grading Around Building.

1. Grade all areas to produce uniform slopes between spot elevations and contours.

2. Control grading within 5 feet of building to prevent water from running

C. General Excess Material.

1. Remove excess material, not required for grading, from the site as directed by CO.

3.5 REPAIR/RESTORATION

A. Contractor shall be responsible for locating and protecting existing underground utilities that are to be preserved for future use.

B. Damage to existing underground utilities.

1. Stop all work associated with damage to existing utilities that are to be preserved for future use, and notify the CO. Repair at Contractor's expense.

3.6 FINISH GRADING

A. All surfaces and slopes shall be shaped to blend with the original ground line, mounded over or smoothed off, raked, and left in a uniform and neat condition.

B. Stockpiled topsoil shall be smoothly distributed over disturbed areas and hand raked to blend with ground line.

C. Slope surface to provide drainage away from structure.

D. Final grade shall be flush with top of slabs to provide accessibility.

E. Surface drainage shall be diverted so that it will not enter into the building area.

3.7 CLEANUP

A. The disturbed area shall be finished to present as near a natural appearance as possible and cleaned up by removing all debris and material not utilized after the installation of the CXT unit has been completed.

3.8 DISPOSAL

A. All unsuitable excavated material, stumps, small limbs, brush, and other construction refuse from this construction project shall be legally disposed of off Government land at a State-approved disposal site.

PART 4 – MEASUREMENT AND PAYMENT

4.1 METHOD OF MEASUREMENT

A. Measurement for the site preparation of the pre-cast concrete toilet buildings will be the lump sum quantity for all work specified in this Section.

4.2 BASIS OF PAYMENT

A. The accepted work will be paid for at the Contract unit price for each item shown on the Schedule of Items.
