About this file
This is a Contractor Project Closeout form issued by the State of Arizona General Services Division for use in finalizing construction contracts and projects. The document serves as a standardized checklist for contractors, architects/engineers, and construction program specialists to ensure all required deliverables and documentation are submitted upon project completion.
Contractors are required to submit two binders with hard copies and two electronic copies on USB thumb drives containing comprehensive project documentation. Required contractor deliverables include Certificates of Substantial and Final Completion, final pay request documentation with unconditional lien releases from all subcontractors and material suppliers, surety consent forms, and evidence of project completion per specifications. Additional requirements encompass record construction documentation such as addenda, RFI logs, approved submittals, change orders, executed contracts, red line drawings in both full-size print and electronic format, and certificates of inspection from relevant authorities. Contractors must also provide maintenance and operations manuals, manufacturer's safety data sheets, a notarized letter certifying use of asbestos-free materials, and warranty documentation. Architects and engineers are responsible for incorporating contractor red line drawings into one hundred percent as-constructed record drawings and specifications, submitting these documents in CAD and PDF formats on flash drives and hard copy prints. All closeout binders and documents remain in General Services Division filing for the duration of the two-year warranty period, after which they are transferred to the State Archivist for storage according to retention procedures.
