About this file
This is a Field Order Directive form issued by the State of Arizona General Services Division for use in managing construction change orders on state projects. The document serves as a template for the Design Professional, Contractor, and Agency Designated Representative to authorize modifications to existing construction contracts. The form allows for documentation of proposed changes including lump sum increases, unit price adjustments, or other cost modifications, as well as changes to the substantial completion timeline. The Field Order Directive requires the contractor and design professional to submit a complete Construction Contractor Change Order Summary Form within a reasonable timeframe following execution, which should include supporting documentation such as drawings, material substitution literature, and detailed cost breakdowns specifying labor, materials, markup, and tax components.
All requirements, specifications, terms, and conditions of the original contract remain unchanged unless explicitly modified through this Field Order process. The directive becomes effective immediately upon signed approval by all three authorized parties: the Contractor, Design Professional, and Agency Designated Representative. No work associated with the change order shall proceed until all parties have executed the document. The form is designed to ensure clear communication and documentation of any contract modifications while maintaining the integrity of the underlying construction agreement and establishing a formal record of authorized changes to project scope or schedule.
