About this file
This is Addendum No. 5 to an Invitation for Bid (IFB) issued by the City of Leesburg, Florida, for the Baker Street Sidewalk Project. The addendum, dated May 22, 2026, provides clarifications to bidders regarding project scope and requirements. The project involves constructing approximately 680 linear feet of 5-foot-wide sidewalk along the north side of Baker Street from 1114 Baker Street to Dixie Avenue, along with associated grading, crosswalk construction, and ADA-compliant pedestrian ramps. The addendum clarifies that as a Design-Build project, the selected contractor team is responsible for evaluating existing field conditions, performing due diligence, and incorporating all necessary design, coordination, and construction activities. The City does not have detailed utility relocation drawings or predefined relocation alignments available; therefore, proposers must conduct their own investigations and include all necessary utility coordination, design, permitting, and construction efforts in their proposals. Driveway adjustments should encompass all modifications needed to accommodate the proposed improvements and maintain safe, functional access, with the Design-Build team determining exact limits, widths, grades, and restoration requirements during design development based on field verification.
The addendum also addresses survey and as-built documentation requirements, clarifying that if the final design does not include new wastewater structures or grading modifications, sealed as-built documentation for those specific elements is not required. However, the Design-Build team must provide record drawings and applicable as-built information for all constructed improvements within the final project scope. All bids are considered acknowledgments of receipt of this addendum. The project is funded through the U.S. Department of Housing and Urban Development (HUD) Community Development Block Grant (CDBG) program, and contractors must be actively registered in the System for Award Management (SAM.gov) at the time of bid submission and throughout contract duration. Contractors must comply with all applicable HUD requirements, including Davis-Bacon Act wage determinations, federal labor standards, and all CDBG program regulations and reporting requirements.

DATE: May 22, 2026 TO: All Interested Bidders and Other Interested Parties FROM: Lissandra Kmoch, Buyer
The City makes the following changes, additions, deletions, information, and/or clarifications and now considers the addendum a part of the IFB Document. Bidders must acknowledge receipt of this addendum by indicating receipt in the space provided on the Proposal Bid Form.
If the bidder has already submitted the Proposal Bid Form to the City’s Procurement Division, the Bidder should revise their electronic submission to include this addendum. In any case, the submission of a bid is an acknowledgment of receipt of the addendum. The City will construe all bids received as though the Bidder has received and acknowledged the addendum.
Below is the City’s reply to RFIs (Request for Information – a/k/a bid inquiries or questions) submitted by interested parties by the posted deadline to submit RFIs.
Q1. What type of utilities are present? What size pipes are present? Where do the utilities get moved to? What does a "driveway adjustment" entail? Demo existing driveways? How wide?
Can you please describe the work you expect to be done and provide a detail drawing? The Bid Documents state "Contractor shall survey as-built elevations of new wastewater structures and grading changes within the Project limits. A copy of the as-built drawings, signed and sealed by a professional surveyor and mapper licensed in the State of Florida, shall be provided by the Contractor." Since there are no new wastewater structures or grade changes within this project, is a professional survey / as-built drawing required?
A1. As this is a Design-Build project, the selected team is responsible for evaluating existing field conditions and incorporating all necessary design, coordination, and construction activities to complete the project improvements.
Currently, the City does not have detailed utility relocation drawings, pipe size inventories, or predefined relocation alignments for the utility conflicts identified in the addendum. Proposers should use available site information, perform due diligence, and include all necessary investigation, coordination, design, permitting, and construction efforts for utility relocations in their proposals.
Driveway adjustments should include all modifications needed to accommodate the proposed improvements and maintain safe, functional access. The Design-Build team will determine the exact limits, widths, grades, and restoration requirements during design development based on field verification and final engineering. Proposals should account for all labor, materials, demolition, restoration, and associated work necessary to complete these adjustments.
For survey and as-built requirements, the City expects record documentation for all improvements constructed as part of the project. If the final design does not include new
