About this file

The document is a Past Performance Questionnaire (PPQ) for a Training and Instructional Support Services contract solicited by the US Patent and Trademark Office (USPTO) under RFP # 1333BJ25R00150009. The PPQ is a detailed evaluation form designed to assess a contractor's past performance across six key areas: Technical/Quality of Service, Schedule/Timeliness, Cost Control, Management/Business Relations, Small Business Subcontracting, and Regulatory Compliance. Each section includes rating options from Exceptional to Unsatisfactory, with requirements for narrative explanations for ratings other than "Not Applicable".

The questionnaire provides comprehensive rating guidelines and requires the past performance reference to complete and email the form directly to specific USPTO contacts (robert.garske@uspto.gov and kelly.gustafson@uspto.gov) with a specific subject line. The PPQ is part of a pre-solicitation for Training and Instructional Support Services, with the goal of capturing detailed performance insights from previous contractors to inform future contract award decisions. The form emphasizes evaluating contractor performance across technical capabilities, operational efficiency, cost management, and compliance with contractual requirements.
