About this file
This is a Quarry/Stockpile Record of Inspection form used to document inspections of quarries and stockpiles for weed control accreditation. The form is designed to evaluate sites based on the presence of noxious weeds and includes four levels of accreditation: Exceeds standards (A), Meets standards (B), Conditional (C), and Not Met (D).
The inspection form requires detailed information about the inspector and quarry/stockpile, including contact information, site location, and material types being inspected (sand/gravel/rock/top soil). The accreditation assessment focuses on the presence of priority and secondary noxious weeds within the quarry site and buffer zones. The form includes sections for documenting noted weeds, suggested treatment and prevention schedules, and past treatment efforts. Requirements specify that inspections must cover surrounding ditches, top soil piles, gravel/sand piles, fence rows, roads, rights-of-way, work areas, storage areas, and buffer zones. The form appears to be related to a Bureau of Land Management (BLM) botany survey contract opportunity.

Level of Accreditation:
See attached list of accreditation standards
(Site = quarry, stockpiled materials, access roads, appropriate buffer zones)
A._____ EXCEEDS standards - No priority or secondary noxious weeds (see attached list) or undesirable plant species present at site or within a buffer zone (from which weed seed could invade quarry).
B._____ MEETS standards- The site contains no priority noxious weeds or significant infestation(s) of secondary weeds (see attached species list) but there may be one or more species from the secondary list located in areas where seed transport into gravel stockpiles is unlikely.
(Noted weeds present)__________________________________________________________________
C._____ CONDITIONAL - The site contains priority noxious weeds in the periphery of the active quarry area, but the risk would be minimal that priority noxious weed seed or plant parts would be transported in gravel taken from the site AND the quarry owner/manager is actively treating existing noxious weed populations to make progress toward meeting or exceeding the accreditation standards.
(Noted secondary weeds present)___ ______ Suggested Treatment & Prevention schedule:__________________________________________________________________
D._____ NOT MET – Site contains noxious weeds in areas where weed seeds would likely be transported in gravel removed from the site.
