About this file
This document is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form issued by the Incorporated County of Los Alamos Procurement Division, located at 101 Camino Entrada, Building 3, Los Alamos, NM 87544. The form is a mandatory certification that potential contractors or bidders must complete to verify their eligibility for a county contract, affirming they have not been debarred, suspended, or otherwise disqualified from public transactions.
The certification requires bidders to attest that within the past three years, they have not been convicted of fraud, antitrust violations, embezzlement, theft, forgery, bribery, or other offenses that would compromise their business integrity. Additionally, bidders must confirm they are not current or former county employees, not related to county employees as an "immediate family member", and have not had previous public contracts terminated for cause or default. The document emphasizes that false statements can result in the rejection of a proposal or rescission of a contract award, and that each certification will be thoroughly reviewed by the county.
