About this file

This is a Business Continuity/Disaster Recovery (BC/DR) Questionnaire form for the Texas Office of the Attorney General (OAG) Service of Process (SoP) Request for Proposal. The form is designed to evaluate vendor compliance and readiness regarding internal recovery strategies, plans, and overall BC/DR program capabilities. The questionnaire contains 12 primary questions addressing regulatory oversight, audit findings, program documentation, management approval, governance structures, and periodic review processes. Vendors are required to confirm whether their organizations maintain written BC/DR policies, have designated oversight personnel, conduct annual plan reviews, and ensure third-party suppliers align with BC/DR objectives. The form also requests identification of any anticipated business disruptions that would require exceptions to planned recovery strategies.

The questionnaire serves as a standard evaluation tool for the OAG to assess vendor readiness for providing Service of Process services across in-state, nationwide, and international locations for the Child Support Division. Responses to the questionnaire are mandatory, with affirmative answers requiring supporting documentation such as regulatory agency names, audit findings, remedial actions, and corrective action implementation status. The form requires vendors to demonstrate the existence of formal governance bodies, senior management sponsorship, and evidence of corrective actions for any identified program deficiencies, ensuring that contracted vendors maintain adequate business continuity capabilities to support the state's critical service delivery operations.
