About this file
This is a Form A Respondent's Execution of Proposal and Assurances document for the Texas Office of the Attorney General (OAG) Service of Process Request for Proposal, which seeks qualified vendors to provide Service of Process services for the Child Support Division in-state, nationwide, and international locations. The document serves as a certification and assurance form that respondents must complete and submit as part of their proposal response. Respondents are required to provide corporate information including Federal Employer ID, Corporate Charter number, entity name, and contact details. Additionally, respondents must disclose the names and Social Security Numbers of all individuals with at least 25% ownership of the business entity. The authorized representative signing the form must provide proof of authorization through submission of a Corporate Board of Director's Resolution or similar document.
The form contains 28 distinct certifications and assurances that respondents must acknowledge, including compliance with antitrust laws, child support obligations, suspension and debarment status, conflicts of interest, E-Verify utilization, Israel boycott prohibitions, cybersecurity training requirements, data security controls, and restrictions on human trafficking and vaccine passport requirements. Respondents certify that all information provided is current, complete, true, and accurate, with acknowledgment that submission of false information may result in contract termination, payment withholding, and liability to the OAG for attorney's fees and costs associated with contract completion. The document emphasizes that failure to provide required ownership information will result in disqualification of the response.
