About this file

This document is Form 4 – Respondent's Statement of Organization for the City of Brownsville Procurement Department, dated October 2025. The form is designed to collect comprehensive organizational information from businesses seeking to participate in city solicitations and cooperative contract purchases requiring City Commission approval. It requires detailed entity information including legal business name, federal and state tax identification numbers, business address, contact details, organizational type, state of incorporation, and business registration status in Texas.

The form mandates ownership disclosure for principals with 10% or more equity interest and includes a critical debarment and suspension certification. Respondents must certify they are not presently debarred by any government entity, have not been convicted of fraud or other integrity-related offenses in the past five years, and agree to immediately notify the city of any such events. The document emphasizes the importance of providing accurate information, warning that false or misleading statements may result in disqualification, contract termination, or potential prosecution under Texas Penal Code for tampering with governmental records.
