About this file
This document is Schedule D - Service Level Agreement for RFP 260000002197 Janitorial Services for the State of Michigan. The agreement establishes performance standards and accountability measures for the contractor across ten designated regions. The contract requires the contractor to meet specific service level requirements including emergency response protocols, staffing availability, equipment and supply provisions, reporting obligations, and snow removal services where applicable. The contractor must respond to emergency calls within two hours, correct unsatisfactory conditions within eight business hours, maintain adequate staffing during designated cleaning times, provide various reports and safety data sheets within specified timeframes, and maintain all necessary equipment and supplies approved by the Program Manager. The agreement also mandates pre-employment background checks and drug testing for all assigned staff prior to deployment at state locations.
The Service Level Agreement establishes financial penalties for non-compliance with specified service standards. Failure to respond to emergency situations results in a $250 invoice credit, while unsatisfactory conditions that are not corrected within eight business hours incur $25 per day for the first day and $100 per day for subsequent days. Inadequate staffing triggers $250 credits for the first instance and $500 for subsequent occurrences by the same employee, with additional penalties for failure to notify of supervisor changes. Late or incomplete reporting and data sheet submission incurs $150 per day credits, and failure to provide adequate supplies results in $150 credits if not remedied within specified timeframes. Snow removal failures result in $25 per day for the first day and $50 per day for additional days. Unverified employee background checks and drug testing incur penalties of $100 per unverified employee per day or 10 percent of the monthly invoice, whichever is greater. The State reserves discretion to apply credits as deductions from payments due to the contractor or as direct payments to the State within 10 days of assessment notice, and may review extenuating circumstances before assessing penalties.
