About this file
This is an attachment to a Request for Proposals (RFP) issued by the State of Utah Division of Purchasing on behalf of the Department of Health and Human Services for pharmaceutical services at the Utah State Developmental Center (USDC). The RFP seeks a single contract award for a five-year term with pricing guaranteed for one year. The document outlines mandatory minimum requirements and technical scoring criteria that vendors must satisfy to be considered for award. Vendors must demonstrate an active professional pharmacy license in Utah and their state of operation, maintain active Medicaid provider enrollment with a valid National Provider Identifier (NPI), and certify capability to bill Medicare, Medicaid, and private insurance plans. These mandatory requirements are evaluated on a pass/fail basis, and vendors must pass all requirements to advance to the technical evaluation stage.
The technical evaluation component includes five scoring criteria assessed on a 0-5 scale by an evaluation committee. Vendors must address pre-authorization processes and rejection notification timelines; delivery schedules and sourcing procedures for out-of-stock items; customer service protocols including tracking systems, error correction procedures, and service response guarantees; technical integration strategies with implementation timelines and State IT resource requirements; and details regarding proposed automated dispensing machine equipment and comprehensive training programs for both vendor staff and USDC personnel. Vendors must achieve a minimum technical score threshold specified in the RFP to advance to the cost evaluation stage, where the highest-scored responsive and responsible vendor will be selected for award.
