About this file

This is an RFP evaluation scoresheet for Pharmaceutical Services (Solicitation #MK27-2) issued by the State of Utah Division of Purchasing and conducted by the Department of Health and Human Services to provide pharmacy services for individuals at the Utah State Developmental Center. The contract is anticipated to result in a single award to the responsive and responsible vendor with the highest evaluation score, with a contract term of five years and a pricing guarantee requirement of one year.

The evaluation methodology assigns points across mandatory requirements, technical criteria, and cost proposals. Mandatory requirements include vendor certification of an active professional pharmacy license in Utah and the vendor's state of operation, active Medicaid provider enrollment with a valid National Provider Identifier (NPI), and capability to bill Medicare, Medicaid, and private insurance plans. Technical evaluation comprises five scored criteria: Pre-Authorization Plan (250 points, 25% weight), Delivery Schedule (200 points, 20% weight), Customer Service Response Plan (250 points, 25% weight), Technical Integration Plan (200 points, 20% weight), and Automated Dispensing Machine capabilities and Training Plan (100 points, 10% weight), for a total of 1,000 technical points with a minimum required threshold of 700 points (70%). Cost proposals represent 30% of total evaluation points (428.6 points maximum), calculated using a formula where the lowest-priced proposal receives 100% of cost points and other proposals receive proportional points based on their price differential. The combined total evaluation is 1,428.6 points, with technical criteria comprising 70% and cost comprising 30% of the overall evaluation.
