About this file
This document is an attachment to a Request for Proposal (RFP) issued by the State of Utah Division of Purchasing on behalf of the Department of Health and Human Services for pharmaceutical services at the Utah State Developmental Center (USDC). The RFP seeks a single contract award for a five-year term to provide comprehensive pharmacy services to individuals served by the department. Vendors must meet all mandatory minimum requirements, including maintaining an active professional pharmacy license in Utah and their state of operation, holding current Medicaid provider enrollment with a valid National Provider Identifier, and demonstrating the capability to bill Medicare, Medicaid, and private insurance plans. Vendors must also achieve a minimum technical score threshold to advance to cost evaluation.
The technical evaluation criteria require vendors to provide detailed responses addressing five key areas: pre-authorization process and turnaround times for claim rejections; delivery schedule strategy and guaranteed timelines for out-of-stock items; customer service protocols including problem tracking, error correction procedures, and maximum wait times; technical integration plan with implementation timeline, testing phases, deployment date, and estimated State IT resource requirements; and automated dispensing machine specifications with comprehensive training curriculum and timelines for both vendor and USDC personnel. Vendor pricing must be guaranteed for a minimum of one year. Scoring will be conducted on a 0-5 scale for each technical criterion, with responses evaluated by an appointed evaluation committee.
