About this file
This is a Cost Sheet Attachment document for pharmaceutical services procurement (MK27-2) issued by the State of Utah Division of Purchasing on behalf of the Department of Health and Human Services for the Utah State Developmental Center. The cost proposal form requests unit pricing for 53 pharmaceutical products and supplies, including medications such as acetaminophen, aspirin, baclofen, various vaccines, and specialized pharmaceuticals like Botox and Dysport. The quantities listed represent approximately one month of orders for evaluation purposes only, with actual quantities and products subject to change. Bidders must enter unit prices only in designated yellow cells, with extended prices and subtotals calculated automatically. The contract is anticipated to be a five-year award to the responsive and responsible vendor achieving the highest overall score, with vendors required to guarantee pricing for a minimum of one year.
Pricing evaluation will be conducted using a cost-based scoring methodology whereby the lowest proposed total cost receives 428.6 cost points, with all other offerors receiving proportional cost points calculated by the formula: Cost Points × (Lowest Proposed Price/Proposed Price). In addition to pharmaceutical products, the cost sheet requests pricing for onsite pharmacy technician services on a monthly basis and quarterly chart-check costs. Vendors must also declare any additional charges not listed on the form. The grand total cost, encompassing all products, services, and additional charges, will serve as the primary evaluation criterion for determining the lowest cost proposal and corresponding cost point allocation.
