About this file
This is a Cost Proposal Form Attachment C for a Social Emotional Wellness (SEW) Curriculum Request for Proposals issued by Jordan School District in Utah. The RFP seeks to establish a contract with a qualified vendor to provide a comprehensive K-12 Social Emotional Wellness Curriculum and related services to the school district. The proposal requires vendors to price a specific evaluation scenario consisting of seven schools with defined enrollment counts: two high schools (Herriman High with 2,560 students and West Jordan High with 1,752 students), two middle schools (Copper Mountain Middle with 1,064 students and Elk Ridge Middle with 1,139 students), and three elementary schools (Oak Leaf Elementary with 572 students, Heartland Elementary with 427 students, and Bastian Elementary with 828 students), totaling 8,342 students across all grade levels K-12.
Offerors must submit all-inclusive pricing in six sections of the cost proposal form. Section 1 requires a Grand Total All-Inclusive Cost for the entire purchasing scenario and per-student pricing for evaluation purposes. Section 2 requires identification of items included in the all-inclusive cost, while Section 3 identifies excluded items that are not required for curriculum functionality. Section 4 permits optional add-ons with separate pricing that do not count toward the Grand Total. Section 5 requires pricing by school type and enrollment range for future contract use, with pricing that must be consistent with Section 1 calculations. Pricing submitted in Section 5 will be incorporated into the resulting contract and utilized by Jordan School District for future purchases throughout the contract term. Section 6 requires vendor certification that all pricing is complete, accurate, and based on the district's specifications. Vendors are explicitly prohibited from submitting reduced pricing for the evaluation scenario while offering higher pricing for future contract purchases. The calculated price per student will be provided for comparison purposes only and will not replace the Grand Total All-Inclusive Cost used for cost scoring.

In Section 2, Offerors must identify what is included in the all-inclusive cost per school and provide the price for each item or service included.
In Section 3, Offerors must identify any items or costs that are not included in the all-inclusive cost per school. Items listed as not included or excluded must not be required for the SEW curriculum to function as proposed.
In Section 4, Offerors may identify optional add-ons that are available for purchase separately and must include pricing for each optional add-on. Optional add-ons must not be required for the SEW curriculum to function as proposed and shall not be included in the Grand Total for the purchasing scenario.
In Section 5, Offerors must provide all-inclusive contract pricing by school type and enrollment range. Pricing submitted in Section 5 shall be included in the resulting contract and may be used by Jordan School District for future purchases throughout the contract term.
