NAME OF CONTRACTOR REQUESTING THE REFERENCE:

The purpose of this questionnaire is to obtain information regarding the quality of the above-named Contractor’s past performance relative to a contract, completed or in progress, at your company/agency. NASA Armstrong Flight Research Center’s Request for Proposal No.

80AFRC18R0001 requires Offerors to identify current or previous customers and solicit their response regarding our performance.

It is requested that the individual(s) responsible for the administrative oversight of the project (e.g.

COR, project manager, etc.) respond to this questionnaire.

Upon completion, please e-mail this questionnaire to: Angela Stinchfield, angela.t.stinchfield@nasa.gov and James G. Williams, james.g.williams-1@nasa.gov on or before February 15, 2018, 10:00 am PT.

The information contained in the completed Past Performance Questionnaire is considered sensitive and cannot be released to us, the Offeror. CONTRACTOR PERFORMANCE

Based on your knowledge of the contract identified above, please provide your assessment of how well the contractor performed on each of the following questions.

1. How consistent is/was the Contractor in understanding the customer’s requirements and providing sufficient staffing?

2. Did the Contractor provide fully trained, qualified and certified (if required) staffing per the requirement?

3. Was the Contractor able to recruit and retain quality personnel in a timely manner?

4. Did the Contractor provide for continuity of services during employee absenteeism and position vacancy?

5. How complete is/was the Contractor in keeping training records, certifications, and related documentation complete, accurate, and current?

6. Was the Contractor able to build effective working relationships with associate contractors, subcontractors and the government in a team environment?

7. How effective is/was the Contractor in identifying, responding and resolving problems?

8. How is/was the Contractor’s emphasis on safety protocol and responsiveness to mishap investigations (if applicable)?

9. Discuss the Contractor’s ability to seamlessly transition or phase-in under a newly awarded contract (if applicable):

10. Discuss the Contractor’s ability to provide sufficient management to ensure effective personnel performance over the course of the contract.

____ Very Good: Very effective performance, fully responsive to contract requirements;

contract requirements accomplished in a timely, efficient, and economical manner for the most part; only minor deficiencies.

____ Good/Satisfactory: Effective performance; fully responsive to contract requirements;

reportable deficiencies, but with little identifiable effect on overall performance.

____ Fair/Marginal: Meets or only slightly exceeds minimum acceptable standards; adequate results; reportable deficiencies with identifiable, but not substantial effects, on overall performance.

____ Poor/Unsatisfactory: Does not meet minimum acceptable standards in one or more areas;

remedial action required in one or more areas, deficiencies in one or more areas which adversely affect overall performance.
