About this file
This is a Foreign Entity Ownership Affidavit required by the State of Florida for Indian River County's Fire/EMS Station #15 construction project. The affidavit must be submitted with every bid, offer, or contract that provides an entity access to personal identifying information. Bidders must attest under penalty of perjury to compliance with Florida Statutes regarding foreign ownership restrictions, including that the entity is not owned by a foreign government of concern, does not have a controlling interest held by a foreign government, and is not organized under or principally based in a foreign country of concern as defined in Sections 287.138 and 692.201 of the Florida Statutes.
The affidavit requires entities to confirm they are not foreign principals, are in compliance with applicable foreign ownership requirements, and, if applicable to real property purchases, are not prohibited foreign principals under Section 692.204. An authorized representative of the entity must execute the affidavit by providing their signature, printed name, title, the entity name, and date. This certification is a mandatory submission requirement for all bidders participating in the Indian River County Fire/EMS Station #15 project, which involves constructing a 9,983 square foot fire station and EMS building in Vero Beach, Florida.
