Centers for Disease Control and Prevention (CDC) Ft. Collins Campus – Building 401 3156 Rampart Road Ft. Collins, CO 80521
I. AMENDMENT #1 INCLUSIONS: SCOPE OF WORK
This Amendment includes changes to the Statement of Work and the Drawings. This Amendment consists of:
A. Revisions to the Statement of Work, all edits from the previous set are highlighted in red and the new S.O.W.
is dated August 21, 2025.
B. Revisions to the Drawings:
1. Drawing G000: Updated Index of Drawings with dates of ASI revisions added. Date of updated
G000: 8/21/25.
2. Drawing A102: Amended with additional scopes of work, including new rooms and walls that are proposed to be covered with the FRP system. Date of updated A102: 8/21/25.
II. SCHEDULE AND CONTRACTOR RESPONSIBILITIES
A. As soon as possible, and as coordinated with the CDC’s OFR Contracting Officer (C.O.), submit a proposal with the proposed construction cost, including all other project and management costs, to include this Amendment’s scope of work. Include breakdowns by discipline, as well as the Construction Schedule for Government review and approval. Provide to the CO a reasonable time frame for when the Cost Proposal can be submitted.
B. Submit all materials and accessories for approval. All adhesives must be approved and certified for use by the FRP panel manufacturer, and be low VOC.
C. Provide all labor, materials, badging support, design, coordination, installation, building occupant signage, and user notifications to make a complete and functional building.
