It requests the recipient to complete and return the questionnaire, which evaluates the contractor's past performance on a previous contract in areas such as quality of service, schedule, cost control, business relations, and management of key personnel. The questionnaire was issued in relation to an upcoming solicitation from the U.S. Department of Health and Human Services (HHS), Administration for Children and Families (ACF), for legal services in support of ACF's Office of Refugee Settlement (ORR) Unaccompanied Children (UC) Program, as outlined in the attached Performance Work Statement. The completed questionnaire is requested to be submitted by a specific deadline via email to the provided address. This is to assist the government in evaluating the contractor's past performance as part of the overall proposal evaluation process for the upcoming solicitation.
