About this file
This document is a Past Performance Questionnaire for a U.S. Coast Guard Operational Logistics Command (LOGCOM) contract for Full Food Services at the Coast Guard Academy in New London, CT. The solicitation (70Z08426RDL950003) seeks to evaluate potential contractors' past performance across multiple performance categories including quality, schedule, cost control, management, and regulatory compliance. The questionnaire allows evaluators to rate contractor performance using a six-point scale from Exceptional (E) to Neutral (N), with detailed assessment criteria covering areas such as quality control, personnel management, problem-solving, regulatory compliance, and overall contract performance.
The document is designed to collect comprehensive performance data from previous contract references, with sections for both the offeror and the respondent to complete. Key elements include documenting contract details like total dollar value, period of performance, contract type (which may include FFP, CR, T&M, IDIQ), and specific performance metrics across multiple evaluation dimensions. The form emphasizes treating the information as "Source Selection Information" and requires responses to be emailed to a specific Coast Guard point of contact, with the ultimate goal of supporting the procurement of full food services for the Coast Guard Academy.
