About this file

This is a Past Performance Questionnaire template used to evaluate contractor performance on completed federal contracts. The form collects comprehensive information about a contractor's past work, including the contractor name, contract number, contract type (Firm Fixed Price, Cost Reimbursement, or Other), performing agency, point of contact, performance location, period of performance, and total dollar value including modifications. The questionnaire requires evaluation of seven performance areas: Quality of Work, Personnel, Subcontractor Management, Business Relations, Timeliness of Performance, Customer Satisfaction, and Cost/Budget Control, with each area rated on a five-point scale (Unsatisfactory, Marginal, Satisfactory, Very Good, or Exceptional).

The evaluation guidelines provide detailed criteria for assessing each performance category. Quality of Work ratings consider compliance with contract requirements, accuracy of reports, personnel appropriateness, and technical excellence. Cost Control evaluations examine whether work stayed within budget, billing accuracy, cost efficiencies, and change orders issued. Timeliness assessments focus on meeting interim milestones, reliability, and on-time completion. Business Relations and Subcontractor Management ratings evaluate effective management, businesslike correspondence, responsiveness, problem notification, cooperation, flexibility, and implementation of small business subcontracting programs. The form concludes with a question regarding whether the evaluator would select the firm again, requiring explanatory comments. This template is designed to be completed by government personnel to document past contractor performance for use in future source selection decisions.
