About this file
This is an Insurance Schedule document for Nevada's Division of Environmental Protection (NDEP), Bureau of Corrective Actions, establishing minimum insurance requirements for contractors providing Environmental Mitigation, Assessment, and Remediation Services under RFP# 70CNR-S3900. The document does not specify project timelines, response dates, due dates, site visits, bidders' meetings, award dates, or contract terms, as it functions solely as an insurance requirements appendix to the broader RFP.
The insurance requirements mandate that contractors maintain Commercial General Liability coverage with a General Aggregate limit of $2,000,000 and Each Occurrence limit of $1,000,000; Automobile Liability with a Combined Single Limit of $1,000,000 for owned, hired, and non-owned vehicles; Workers' Compensation at statutory levels with Employers' Liability limits of $100,000 per accident and $500,000 policy limit; and Contractor's Pollution Liability (including Errors & Omissions) with $1,000,000 per occurrence and $2,000,000 general aggregate limits. All policies must name the State of Nevada as an additional insured, be primary and non-contributory to other coverage, be issued by insurers rated A-VII or better by A.M. Best, and remain in effect throughout the contract duration and any applicable warranty periods. Contractors must provide certificates of insurance on ACORD forms prior to work commencement and maintain thirty days' notice of cancellation (ten days for non-payment), with failure to maintain required coverage constituting material breach of contract. Subcontractors must either be included as additional insureds on the contractor's policies or provide separate certificates meeting identical minimum requirements.
