About this file

This is a Request for Proposal evaluation scoresheet for Pest Control Services issued by the State of Utah Division of Purchasing under Solicitation Number SB26-4. The RFP seeks vendors to provide comprehensive pest control services including general pest control, bird deterrents, and wildlife solutions. The contract term is five years, with vendors required to guarantee pricing for a minimum of 12 months. The evaluation process is structured in two separate categories: Category #1 for General Pest Control services and Category #2 for Wildlife and Bird Resources services, allowing for multiple awards across both categories.

The evaluation scoresheet establishes a two-stage selection process with mandatory minimum requirements followed by technical evaluation. Vendors must score at least 480 of 800 available technical points (60 percent) to advance to cost evaluation. The technical evaluation comprises 800 points across prerequisite questions and category-specific criteria addressing staffing, service delivery, regulatory compliance, certifications, equipment capabilities, and experience. Cost evaluation represents 200 points per category, with the lowest-priced vendor receiving 100 percent of cost points and other vendors receiving proportional points based on the formula: Cost Points × (Lowest Proposed Price/Proposed Price). Vendors must achieve a combined total of at least 600 of 1,000 available points (technical plus cost) to be awarded a contract. An administrative fee of 0.75 percent is applicable to all contract awards.
