About this file

This cost sheet attachment is a bidding document for the State of Utah Division of Purchasing's Pest Control Services RFP, which seeks vendors to provide general pest control, wildlife control, and bird deterrent services. Vendors must complete pricing information for one or both service categories: Category #1 (General Pest Control Solutions) covering treatments such as general pest services, exterior barrier treatments, preventative maintenance, cockroach control, and bed bug treatment, along with labor costs for various technician levels; and Category #2 (Wildlife and Bird Control Solutions) encompassing wildlife control technician services, field technician hours, bird deterrent pricing, and inspection consultation fees. The contract term is five years with guaranteed pricing for the initial twelve months. Vendors must submit completed cost sheets with all required pricing fields, including any mileage fees beyond forty miles from the company shop and optional value-added services.

The cost evaluation methodology awards 200 points per category to the vendor with the lowest proposed price, with all other vendors receiving proportional points based on the formula: Cost Points × (Lowest Proposed Price/Proposed Price). Vendors must achieve a minimum of 480 points in technical evaluation to advance to cost evaluation, and a combined total of 600 points from technical and cost evaluations to be awarded a contract. An administrative fee of 0.75% applies to all awarded contracts. The cost sheet requires vendors to identify company name, contact information, and pricing for all services they intend to offer, with clear notation that any service fees not listed in the pricing charts cannot be charged to users.
