About this file

This is a Past Performance Reference Sheet template for federal contract solicitation FA4626-26-R-0008. The form is to be completed by reference organizations that have direct experience with a contractor's performance on previous projects. References must be submitted as part of Volume II—Past Performance of the contractor's proposal by July 30, 2026, at 2:00 p.m. Mountain Daylight Time.

The document requires contractors to provide between three and five recent and relevant past performance references. "Recent" is defined as projects completed within six years of the solicitation release date, and "relevant" encompasses design-build and design-bid-build heavy civil construction projects for repair, alteration, and demolition of transportation infrastructure, utilities, and energy systems (communication, electricity, natural gas, sewer, water) valued between $100,000 and $3,000,000. For each reference, the form collects contract/task order number and project title, dollar value, performance dates, customer name and point of contact information, competitive award status, contractor's role (prime, subcontractor, or key personnel), subcontracting arrangements, work description, and specialized knowledge or equipment utilized. The reference organization rates the contractor across 22 performance criteria using a six-point scale (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable), with supporting information required for each rating. Final questions address overall performance rating, cure or show cause notices, and willingness to award future contracts.
