About this file

This cost sheet attachment is part of an RFP issued by the State of Utah Division of Purchasing on behalf of the Department of Corrections to procure motion picture public performance licenses for correctional facilities throughout the state. The procurement seeks a single vendor to provide licenses covering motion pictures shown in prisons, community correctional centers, and other correctional facilities. The contract term is five years, with vendors required to guarantee pricing for the first two years. The cost evaluation is based on an estimated inmate population of 6,000 for scoring purposes only, though actual billing will reflect actual licensed inmate populations during the contract term.

Offerors must submit per-inmate annual rates for each of the five contract years on the provided cost sheet, with all yellow-highlighted cells requiring input. The submitted costs must be inclusive of motion picture licenses per inmate per year and must allow inmate access to both group showings and live streaming daily. The total five-year evaluated cost will be calculated automatically by the cost sheet formula and used for scoring in conjunction with technical criteria, with the responsive and responsible offeror achieving the highest combined score receiving the award. Offerors must not alter any formulas in the cost sheet, and any submission containing formula alterations will be deemed unresponsive. The Department notes that actual inmate populations may vary annually and does not guarantee the estimated 6,000-inmate volume.
