About this file

This is an evaluation score sheet for a Request for Statement of Qualifications (RFSQ) issued by the Utah State Board of Education (USBE) to establish a prequalified vendor list of Continuous Improvement Experts (CIE) to serve USBE-designated Elevate and Springboard schools. The RFSQ seeks to add new vendors to the prequalified list and recertify current vendors who must resubmit complete responses to verify continued compliance with minimum mandatory requirements. The evaluation criteria consist of eight scored sections weighted across technical evaluation, with scoring ranges from 0 to 10 points per section and a total possible score of 650 points. A minimum threshold of 75 percent of total points (487.5 points) is required to pass evaluation. The approved vendor list operates on an indeterminate term, with USBE conducting verification that vendors continue to meet applicable requirements no less frequently than every 18 months. The state retains the right to reopen the solicitation to accept additional vendors at any time during the term.

The evaluation criteria include mandatory minimum requirements and scoreable technical criteria addressing vendor experience in coaching school administrators on data-driven improvement plans, designing and implementing data-driven instructional systems, protecting student-level data, coordinating services with other providers, delivering professional development in instructional effectiveness, demonstrating credible track records of improving student academic achievement, and willingness to partner with any Elevate or Springboard school regardless of location. Vendors must provide resumes and CVs for proposed staff and demonstrate compliance with Section 504 and 508 accessibility requirements. Individual Elevate and Springboard local school boards will independently contract with vendors on the approved list, with all expenses including travel to be determined by the contracting school boards.
